MMP-9、Snail蛋白在肝细胞癌联合表达与术后复发的关系

摘　　要：目的研究基质金属蛋白酶-9(MMP-9)和锌指转录因子(Snail)在肝细胞癌的表达,分析两者联合表达与术后复发的关系。方法收集手术切除后经病理证实的肝细胞癌组织石蜡标本50例及其患者的临床资料、手术记录和随访记录,采用免疫组化检测MMP-9、Snail在肝细胞癌组织的表达,根据表达结果分为联合阳性表达组和阴性表达组,分析两者联合表达的相互关系以及与术后复发的关系。结果 MMP-9表达主要定位于细胞质,Snail表达主要定位于细胞核,部分伴有细胞质表达。MMP-9、Snail在HCC中联合表达率为64%,联合阴性表达率为24%(P<0.05)。MMP-9和Snail在HCC中的表达密切相关(Kappa值=0.72;P<0.01)。两者联合表达组的平均复发时间(9.9±0.6)个月,中位复发时间(9.0±0.8)个月,术后1年复发率79.4%,而两者联合阴性表达组术后平均复发时间(20.9±1.2)个月,中位复发时间(20.0±0.82)个月,术后1年复发率8.3%,两者差异具有统计学意义(P<0.01)。结论 MMP-9和Snail在HCC癌组织的侵袭与转移中可能存在协同关系,两者可以作为评估肝细胞癌恶性程度、术后复发及预后的指标。Objective To investigate the co-expression of MMP-9 and Snail in hepatocellular carcinomatissues, and to analyze the correlation of the two biological indieators with the recurrence after surgery. Methods Fifty cases of pathology specimens were collected, which were selected from hepatocellular carcinoma patients after surgical resection and pathological diagnosis. Paraffin blocks contained cancer and adjacent tissue. Immuno- histochemistry was used to detect the co-expression of MMP-9 and Snail in all cases of HCC tissues, then the correlation between co-expression and recurrence after surgery were analyzed. Results The co-expression rates of MMP-9 and Snail were 64% in HCC tissues, their negative co-expression rates were 24%, MMP-9 and Snail co-expressions had closely relations in HCC tissues each other, their correlations bad statistic significance （Kappa=0.72, P〈0.05）. The average recurrence time in co-expression group means of MMP-9 and Snail in HCC was （9.9 ±0.6） months, the median time was （9.0± 0.8） months; the recurrece rate was 79.4% 1 year postoperation. The average recurrence time in negative co-expression group means of MMP-9 and Snail was （20.9±1.2） months, the median time was （20.0 ± 0.8） months; the recurrece rate was 8.3% 1 year postoperation, there was significant difference between two groups （P〈0.01）. Conclusion MMP-9 and Snail may be have conspiracy relation in tumor formation and progress each other. The two biological indicators can act as the evaluation index of malignance, recurrence and prognosis in HCC patients after surgery as well.
