磷酸肌酸钠对胃癌术后疲劳综合征患者免疫功能的影响

摘　　要：目的：观察磷酸肌酸钠对胃癌术后疲劳综合征患者免疫功能的影响。方法：将165例胃癌患者分为两组，在手术后1～3 d治疗组85例静脉滴注磷酸肌酸钠，对照组80例静脉滴注等容积生理盐水；记录术前及术后第1、3、5、9 d疲劳评分（VAS评分），检测术前及术后第1、3、5、9 d两组免疫球蛋白IgA、IgG、IgM及免疫细胞亚群CD3＋、CD4＋、CD4＋/CD8＋水平。结果：两组患者术后VAS评分较术前显著升高，治疗组升高程度低于对照组（P＜0.05）；术后血清IgA、IgG、IgM、CD3＋、CD4＋、CD4＋/CD8＋水平均明显下降，对照组降低更为明显（P＜0.05）。结论：磷酸肌酸钠能够明显减轻患者胃癌术后疲劳综合征，改善免疫状况，有利于改善生活质量。Objective To investigate the effect and mechanism of sodium creatine phosphate on immunolog-ic function in patients with postoperative fatigue syndrome （POFS） after radical resection of gastric cancer. Methods 165 cases with gastric carcinoma were randomly divided into treatment group （n=85） and control group （n=80）. Intravenous drip of sodium creatine phosphate was given after anaesthesia induction and 1~3 days after operation in treatment group,and normal saline was received in control group instead. After opera-tion, the two group patients were all treated with the same enteral and parenteral nutrition. Fatigue score（VAS score） was measured on the day before the operation,the first,third, fifth and ninth day after operation.The se-rum IgA, IgG, IgM and immune cell subsets（CD3＋, CD4＋, CD4＋/CD8＋） were detected at the same time. Results VAS scores was significantly higher postoperative than preoperative in two groups, while VAS scores in treat-ment group were lower than those in control group （P〈0.05）. Levels of serum IgA, IgG, IgM, CD3＋, CD4＋, CD4＋/CD8＋ decreased significantly postoperative than preop-erative in two groups （P〈0.05）, while the indexes in control group were lower than in treatment group （P〈0.05）. Conclusion Sodium creatine phosphate can alleviate postoperative fatigue syndrome, improve the immune state and promote early rehabilitation of patients.
