12导联动态心电图ST段及最大ST/HR对冠心病的诊断分析

摘　　要：目的探讨12导联动态心电图ST段、最大ST/HR对冠心病的诊断价值。方法通过冠状动脉造影证实冠心病组150例,非冠心病组50例,根据冠脉造影病变情况分为单支病变组66例,多支病变组84例,根据ST段阳性组106例,ST段阴性组44例,最大ST/HR阳性组130例,最大ST/HR阴性组70例。结果 ST段诊断冠心病的敏感性为70.7%,特异性为64%,准确性为69%,均低于最大ST/HR 83.3%、90%、85%,ST段诊断冠脉造影病变支数的敏感性为54.5%,特异性为83.3%,准确性为70.7%,均低于最大ST/HR 70%、90%、83.3%,差异均有统计学意义(P<0.05)。结论 12导联动态心电图最大ST/HR对冠心病诊断价值相对较高,且随着冠脉病变程度的加重,其敏感性明显增加。Objective To approach diagnosis of coronary heart disease ST segment and the maximum ST/HR by 12 lead dynamic electrocardiogram. Methods CHD group 150 eases and Non CHD group 50 cases were confirmed by coronary angiography,single branch lesion group 66 cases and multi branch lesion group 84 cases were confirmed by coronary angiography,ST positive group 106 cases and ST negative group 44 cases were confirmed,maximum ST/HR positive group 130 cases and maximum ST/HR negative group 70 cases were confirmed. Results The sensibility 70.7% ,specificity 64% ,accuracy 69% of coronary heart disease by ST diagnosis were lower than 83.3%,90%,85% of coronary heart disease by maximum ST/HR, sensibility 54.5%,specificity 83.3%,accuracy 70.7% of coronary artery lesion by Diagno- sis of ST segment were lower than 70%,90%,83.3% coronary artery lesion by maximum ST/HR diagnosis,the difference were statistical significant （P〈0.05）. Conclusion The diagnostic value of the maximum ST/HR are higher,the increase of degree of coronary artery lesions ant（P〈0.05）, the sensitivity increased significantly.
