Correlation of Shear-wave Elastography with Clinicopathological and Immunohistochemical Feature in Breast Invasive Carcinoma

摘  要:目的应用实时剪切波弹性成像(SWE)探讨乳腺浸润性导管癌硬度与其临床病理及免疫组化特征的相关性。方法选择经病理证实的乳腺浸润性导管癌患者116例,术前均行常规超声及SWE检查,记录其弹性平均值(Emean);术后记录其临床病理及免疫组化特征,并分析其与Emean的相关性。结果单因素分析结果显示:肿瘤大小、腋窝淋巴结转移、组织分级、临床分期、ER、C-erbB-2、ki67表达与乳腺浸润性导管癌Emean具有相关性。多因素分析结果显示:肿瘤大小、组织分级、临床分期、ki67表达与乳腺浸润性导管癌Emean具有独立相关性。结论乳腺浸润性导管癌硬度与临床病理特点具有显著相关性,乳腺癌的生物学特性影响其硬度的改变。Objective To compare the mean elasticity value, as measured by shear-wave elastography (SWE), with ehnicopathological and immu- nohistochemical feature of breast invasive ductal carcinoma. Methods SWE was performed on 116 cases of breast invasive ductal carcinoma before operation. Mean elasticity values in kPa were measured for each lesion. Record the clinicopathological and immunohistochemical features, and ana- lyze its correlation with mean elasticity value. Results The result of one-way analysis variance showed that the tumor size, histological grade, axillar lymph node slate, clinical tumor stage and the expression of ER, C-erbB-2 and ki67, were related to the mean elasticity value of breast invasive duc- tal carcinoma. The result of mul tivative regression showed that the tumor size, histological grade, clinical tumor stage and the expression of ki67 were independently correlated with the mean elasticity value. Conclusion The clinicopathological feature of breast invasive ductal carcinoma showed significant association with the mean elasticity. Biological characteristics of breast cancer influenced the change of elasticity value.
