胃癌患者肿瘤组织和血清中糖类抗原含量及其临床意义

摘　　要：目的探讨胃癌患者肿瘤组织和血清中糖类抗原含量及其临床意义。方法所有患者均由胃镜及病理诊断确诊,胃癌组33例,对照组(浅表胃炎组)30例。在胃镜检查前抽取患者空腹血2ml,检测血清中糖类抗原CA19-9和CA242的含量。两组患者均行电子胃镜检查,经胃镜取2块病变组织,检测两组患者病变组织中CA19-9和CA242的含量,对两组患者病变组织及血清中CA19-9和CA242的含量及灵敏度、特异度、诊断效率分别进行比较。结果 CA19-9和CA242在胃癌组织中的含量明显高于血清中的含量,差异有统计学意义(P<0.01)。肿瘤组织中CA19-9和CA242检测对胃癌诊断的灵敏度分别为81.3%和84.4%,特异度分别为76.7%和83.3%。血清中CA19-9和CA242检测对胃癌诊断的灵敏度分别为40.6%和46.9%,特异度分别为86.7%和93.3%。肿瘤组织中CA19-9和CA242的诊断效率为62.3%和70.3%,血清中CA19-9和CA242的诊断效率为35.2%和43.8%。结论肿瘤组织中CA19-9和CA242表达对胃癌诊断的灵敏度高,可提高早期胃癌的诊断效率;而血清中CA19-9和CA242表达对胃癌诊断的灵敏度低,对早期胃癌的诊断效率低。Objective To study the clinical significance of carbohydrate antigen determinations in the disease tissue fluid for gastric cancer. Methods Sixty-three patients diagnosed by gastroscopy and pa- thology were divided into two groups, 33 patients in group of gastric cancer, and 30 patients in control group（ CSG group）, the two groups of patients were drawed 2ml blood before gastroscopy check. The ser- um concentrations of CA19-9,CA242 in the two groups of patients were measured. The two pieces of small specimens（ diameter cf about 0.6cm） obtained by biopsyforceps from each patient were put in lml 5% Na2 EDTA solution, and digested and dissociated, The tissue fluid contents of CA19-9 ,CA242 in the two groups were measured and compared, compared with the serum contents of them. The sensitiveties,the specifici- ties ,the diagnostic effieiencyin the disease tissue f^uid and the serum were respectively compared. Results The contents of CA19-9,CA242 in the disease tissue fluid increased significantly than the contents of them in the serum for patients with gastric cancer（P 〈 0. 01 ）. The sensitiveties of CA19-9 ,CA242 in the disease tissue fluid for gastric cancer were respectively 81.3% ,84. 4%. The specificities of them were respectively 76. 7% ,83.3%. The sensitiveties of CA19-9, CA242 in the serum for gastric cancer were respectively 40. 6% ,46. 9%. The specificities of them were respectively 86. 7% ,93. 3%.
