摘 要:目的:以赣南地区单纯性肥胖儿童为研究对象,探讨血浆同型半胱氨酸(Hcy)、D-二聚体(D-D)水平变化与血压、体重指数(BMI)、血脂代谢的相关性和临床意义。方法:30例单纯性肥胖患儿及30例健康体检儿童,采用问卷调查及体格测量方法收集入选儿童人口学资料(年龄、血压、BMI、生活习惯、疾病遗传史等),同时取禁食12 h儿童以上的静脉血,检测血浆Hcy、D-D、血浆甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白(HDL-Ch)、低密度脂蛋白(LDL-Ch)水平。结果:两组不同性别儿童血浆Hcy、D-D浓度比较无统计学差异(P>0.05)。与健康非肥胖儿童相比,单纯性肥胖儿童血压、BMI、Hcy、D-D、TG、TC、LDL-Ch水平明显增高(P<0.05),HDL-Ch浓度明显降低(P<0.01)。肥胖儿童随着肥胖程度的增加,血浆Hcy,TG、TC、LDL-Ch浓度逐渐增加(P<0.05)。肥胖组BMI与血压、TG、TC水平呈正相关,血浆Hcy与血压、BMI、TG、TC水平呈正相关,D-D与HDL-Ch水平呈正相关。结论:赣南地区单纯性肥胖儿童有明显的血压、脂质代谢异常,易发生动脉粥样硬化性疾病。单纯性肥胖儿童血浆Hcy、D-D明显增高,均可作为反映儿童单纯性肥胖症心血管严重事件高风险的较敏感、可靠的指标,具有一定的临床意义。Objective: To explore the clinical significances and correlation between changes of plasma homocysteic acid (Hcy) , D- dimer (D -D) levels and blood pressure, body mass index (BMI), blood lipid metabolism of children with simple obesity in south of Jiangxi province. Methods: Thirty children with simple obesity and thirty healthy children receiving physical examination in the hospital were selected as study group and control group, respectively; the demographic data (age, blood pressure, BMI, living habits, history of ge- netic diseases) of children in the two groups were collected by questionnaire investigation and physical measurement ; after absolute diet more than 12 hours, the venous blood specimens were abstracted to detect the plasma levels of Hcy, D- D, triglycerides (TG), total cholesterol ( TC), high - density lipoprotein cholesterol ( HDL - Ch) and low density lipoprotein cholesterol ( LDL - Ch) . Results : There was no sta- tistically significant difference in plasma Hcy and D - D concentrations between the two groups ( P 〉 0. 05 ) . Compared with control group, blood pressure, BMI, Hey, D - D, TG, TC and LDL - Ch levels in study group increased significantly ( P 〈 0. 05 ) , HDL - Ch concentra- tion decreased significantly (P 〈 0. 01 ) . The concentrations of plasma Hcy, TG , TC and LDL - Ch increased gradually with the aggrava- tion of obesity in study group ( P 〈 0.05 ) . In study group, BMI was positively correlated with blood pressure, TG and TC, plasma Hcy was positively correlated with blood pressure, BMI, TG and TC. D- D was positively correlated with HDL- Ch level. Conclusion: There are significant abnormalities of blood pressure and fat metabolism in children with simple obesity in south of Jiangxi province, so atherosclerosis occurs easily in these obese children.
