非小细胞肺癌患者血浆D-二聚体水平对预后评估的意义

摘　　要：目的探讨非小细胞肺癌(NSCLC)患者手术前后血浆D-二聚体(D-D)水平的变化及与预后相关性。方法收集77例初诊NSCLC患者及98例体检健康者血浆标本。检测NSCLC患者手术前后血浆D-D的水平变化,并探讨其与临床分期、组织分化程度的关系;Kaplan-Meier生存曲线分析术前血浆D-D水平与NSCLC患者发生死亡事件的关系。结果 NSCLC患者术前血浆D-D水平为(1.04±0.71)mg/L,显著高于术后第9天的(0.62±0.47)mg/L(t=4.350,P<0.01);也高于健康对照组的0.37±0.11 mg/L(t=9.294,P<0.01)。初诊NSCLC患者血浆D-D水平随着肿瘤临床分期递进而增加(F=9.028,P<0.01),而与肿瘤组织分化程度无明显关系(F=0.532,P>0.05)。血浆D-D为≤0.87 mg/L的NSCLC患者预后明显优于D-D>0.87 mg/L的患者(χ2=9.72,P<0.01)。结论 D-D水平对NSCLC患者预后评估有一定价值。Objective To evaluate the prognostic value of plasma D-dimer for non-small-cell lung cancer （NSCLC）. Methods The clinical data from 77 NSCLC patients and 98 healthy controls were retrospectively analyzed. The patients＇ clinical and laboratory infor- mation including the baseline and postoperative 9＇h day plasma D-dimer levels, tumor stage and differentiation grade were extracted. The correlation between preoperative plasma D-dimer level and clinical characteristics of NLCLC was analyzed. The relationship be- tween baseline plasma D-dimer level and the fatality was analyzed by Kaplan-Meier survive curve. Results The plasma D-dimer level before operation in NSCLC patients was significantly higher than that on the 9^th day after operation and that in healthy controls （t = 4.350, P 〈 0.01 ）. The baseline plasma D-dimer level in NSCLC patients increased with the advance of tumor grade （ t = 9. 294, P 〈 0.01 ）, but was not related to the differentiation grade of NSCLC （ F = 0.532, P = 0.53 ）. The fatality of NSCLC patients with plasma D-dimer level ≤ 0.87 mg/L was significant lower than that with plasma D-dimer level 〉 0.87 mg/L （X2 = 9.72, P 〈 0.01 ）. Conclu- sion Plasma D-dimer is a useful marker to estimate the prognosis of NSCLC.
