摘  要:目的对比埋植型与非埋植型种植体在义齿修复受载后,种植体骨界面白细胞介素-1β(interleukin-1β,IL-1β)表达的变化。方法选用健康成年Beagle犬8只,于每只实验犬下颌左侧植入埋植型种植体3枚,右侧植入非埋植型种植体3枚,共植入48枚种植体。双侧第1个种植体均不修复,作为对照组,其余种植体均行铸造全冠修复,作为受载组。实验犬分别在加载2、4、8、12周时各处死2只。采用免疫组织化学方法,对修复后不同时期埋植型与非埋植型种植体的种植体骨界面IL-1β的表达进行比较,并对结果进行统计学分析。结果种植义齿修复受载2周时种植体骨界面IL-1β的表达较对照组增高;4周时达高峰,与对照组差异有统计学意义(P<0.05);8周时表达开始下降,但仍高于对照组;12周时恢复到接近对照组水平。埋植型和非埋植型种植体在修复后受载的4个检测时间点种植体骨界面IL-1β水平的差异均无统计学意义(P>0.05)。结论埋植型和非埋植型种植体在义齿修复受载后,所承受的机械负荷均能经过种植体传递至周围骨组织,刺激种植体骨界面IL-1β表达的变化,但二者的表达变化无显著差异。Objective To discuss the changes in the expression of interleukin -1 β( IL-1 β) during implant-bone interlace remodeling after loading of submerged and nonsubmerged implant dentures. Methods Model was constructed by using 8 healthy and adult beagle dogs. Three submerged and three nonsubmerged implants were separately implanted into the left and right side of every beagle dog' s mandible. Fixed metal full crown was used to restore the submerged and nonsubmerged implant denture, except the first submerged and nonsubmerged implant of every beagle dog without being restored as control group. The experimental dogs were sacrificed at 2 weeks, 4 weeks, 8 weeks and 12 weeks after loading separately. Dynamic expression of IL-1β in the implant-bone interface was observed using the method of immunohistochemistry and the results were statistically analyzed. Results In the group after loading of implant denture, the expression of IL-1β in the implant-bone interface boosted clearly than the control group with unloading. 2 weeks after loading, the expression of IL-1β in the interface was observed obviously, and made a statistical difference between experimental and control groups, g weeks after loading, the expression of IL-1β was obviously higher than that in the control group and reached peak. When 8 weeks after loading, the expression of IL-1β began to remit, but also higher than that in the control group clearly. It came down to the level of the control group after loading of 12 weeks. No obvious difference of the expression of IL-113 in the implant-bone interface was observed between the submerged and nonsubmerged implant denture. Conclusion For both the submerged and nonsubmerged implant denture, the mechanical stress which transferred from implant to the surrounding bone tissue after loading could excite the expression of IL-1β in the implant-bone interface and promote the interface remodeling.
