摘 要:目的:应用Johnston分析法研究青少年病例的生长发育对正畸治疗后下颌骨旋转量评价的影响。方法:选取60例处于生长发育高峰期的正畸完成病例,用Johnston分析法描记并重叠治疗前后的头颅侧位片,比较下颌骨整体旋转、基质内旋转、基质旋转的变化。结果:治疗后各类病例下颌骨均未发生明显的整体旋转,但下颌骨下缘外形有明显改建,基质内旋转呈顺时针旋转倾向,下颌平面角的变化值与下颌骨整体旋转值之间有显著性差异;安氏Ⅱ类及安氏Ⅲ类错牙合病例治疗后下颌平面角增大。结论:评价青春发育高峰期正畸病例治疗前后下颌骨的旋转时,应考虑下颌骨发育的影响,以下颌平面角的变化来评价不够可靠,尤其是安氏Ⅱ类及安氏Ⅲ类错牙合病例,应用Johnston头影测量法更可靠。Objective: To evaluate the influence of growth and development on the measurement of mandible rota- tion in adolescents after orthodontic treatment by Johnston analysis. Methods: 60 finished adolescent cases were chosen, lateral cephalometric radiograph were taken before and after treatment. All radiographs were traced and an- alyzed by Johnston cephalometric method. The mandibular total rotation, matrix rotation and intramatrix rotation changes were compared. Results: No significant mandibular total rotation was found after treatment. The shape of mandibular hemline changed significantly after treatment. The mandibular matrix rotation showed a clockwise tend- ency. Significant differences were found between SN--MP changes and mandibular total rotation changes. The SN --MP increased in Angle Class ]] and Class I]I cases after treatment. Conclusion: It is unreliable to evaluate the mandibular rotation in adolescent case before and after treatment by SN--MP change, the growth and development of mandibular bone should be taken into consideration, especially in Angle Class ]] and Class m malocclusion cases, Johnston analysis is more reliable.
