鸦胆子油乳增强宫颈癌调强及^(192)Ir后装放疗效果的临床观察

摘　　要：目的观察中药鸦胆子油乳联合调强放疗(IMRT)及192Ir腔内后装治疗宫颈癌的临床效果。方法将68例宫颈癌患者随机分为2组,对照组34例行全程IMRT联合腔内后装治疗,采用6 MVX线调强放疗平均剂量54.5 Gy,1.8~2.2 Gy/次,每周5次,放疗30 Gy后开始行腔内后装治疗,后装照射共给5次,每周1次,每次6 Gy,治疗当日不进行外照射。研究组34例行全程鸦胆子油乳联合IMRT及腔内后装治疗,在对照组治疗基础上另加用鸦胆子油乳注射液30 mL进行治疗,1次/d,连续使用21 d。观察并比较2组近期有效率、局部控制率、生存率及生存质量等。结果治疗后研究组KPS评分较治疗前显著升高(P<0.05),而对照组显著降低(P<0.05),2组比较有显著性差异(P>0.05),研究组与对照组近期有效率分别为91%和88%(P>0.05),1 a控制率分别为71%和79%(P<0.05),1 a生存率分别为82%和94%(P<0.05);治疗后研究组血液流变学8项指标均较治疗前明显下降(P均<0.05),而对照组无显著性差异(P均>0.05),2组比较有显著性差异(P<0.05)。结论鸦胆子油乳可通过改善患者血液系统微环境,增加氧供给来提高宫颈癌细胞对IMRT和腔内后装放疗的敏感性,增强宫颈癌放疗效果,并进一步提高生存期和生存质量。Objective It is to observe the clinical effect of bruceae oil emulsion combined with intensity modulated radiation therapy （IMRT） and 192Ir after loading intracavitary brach therapy in patients with uterine cervical cancer.Methods Sixty-eight cervical cancer patients were divided randomly into two groups：the control group（34 cases）and the study group （34 cases）.Both groups received IMRT plus intracavitary brachytherapy （1.8-2.2 Gy per fraction,5fractions per week,with an average dose of 54.5 Gy）.Intracavitary brach therapy was used to a total dose of 24-30 Gy at 6 Gy per fraction once a week after the dose of IMRT reached 30 Gy.Besides,the study group received 30 mL of bruceae oil emulsion once a day for 21 days.The recent effective rate,local control rate,survival rate and survival quality in both groups were observed and compared.Results After treatment,the KPS increased and eight BRI decreased in the study group （P 〈 0.05）,while in the control group,the KPS decreased（P 〈 0.05）and eight BRI kept unchangeable（P 〉 0.05）,the difference between the two groups after treatment was statistically significant.The recent effective rate of study group and control group were 91% and 88% respectively（P 〉0.05）,the one year control rate were 71% and 79% （P 〈 0.05）,the one year survival rate were 82% and 94% （P 〈0.05）.After treatment,8 hemorheology indexes in study group were obviously lower than that before treatment,but no obvious change in control group.There were significant differences in the indexes between both groups.Conclusion Bruceae oil emulsion could enhance the sensitivity of cervix cancer cells to IMRT and intracavitary brachtherapy through improving the micro environment of hematological system and increasing oxygen supply,which due to the better life time and quality.
