结直肠癌及癌前病变组织中hTERT表达及其临床意义

摘　　要：[目的]探讨结直肠癌及癌前病变组织中人端粒酶逆转录酶(human telomerase reverse transcriptase,hTERT)的表达及其临床意义。[方法]以免疫组化法检测95例结直肠癌组织、30例结直肠腺瘤息肉组织及30例正常结直肠黏膜组织中hTERT的表达;分析结直肠癌组织中hTERT表达与肿瘤病理特征及预后的关系。[结果]结直肠癌组组织中hTERT表达阳性率和相对含量均显著高于结直肠腺瘤息肉组织和正常结直肠黏膜组织;腺瘤息肉组组织中hTERT表达阳性率和相对含量亦明显高于正常结直肠黏膜组织(P<0.05或<0.01)。结直肠癌患者组组织中hTERT表达相对含量与肿瘤分化程度、浸润深度、淋巴结转移及TNM分期均呈正相关(P<0.05或<0.01),与术后复发呈正相关(P<0.01),与术后生存期呈负相关(P<0.01)。[结论]结直肠癌组织中hTERT表达量与结直肠癌重要病理特征相关,可成为评价结直肠癌预后的指标;结直肠腺瘤息肉组织中hTERT表达升高可能与结直肠腺瘤进展为癌相关。[Objective]To evaluate the expression and clinical significance of human telomerase reverse transcriptase（hTERT）in colorectal cancer and its precancerous lesion. I-Methods]The levels of hTERT ex- pression were detected by immunohistochemistry in colorectal cancers（n= 95）, colorectal adenomatous pol- yposis（n=30）and normal colorectal tissues（n=30）. The relationship between the expression of hTERT in colorectal cancer tissues and the pathologic features and prognosis were analyzed. [Results]The positive rate of hTERT expression and comparative content of hTERT in colorectal cancer tissues were significantly higher than those in the adenomatous polyposis and normal colorectal tissues. The positive rate of hTERT expression and comparative content of hTERT in adenomatous polyposis were also significantly higher than those in normal colorectal tissues（P〈0. 05 or 〈0. 01）. The comparative content of hTERT in colorectal cancer tissues were positively related with the differentiated degree of tumor, depth of invasion, lymph node metastasis,distant metastasis and the postoperative recurrence（P〈0. 05 or 〈0. 01）, and were negatively related with the postoperative survival（P〈0. 05）. [Conclusion]The levels of hTERT expression in colorectal cancer tissues are relative with the important pathologic features, and can be a valuable clinical factor for evaluation of postoperative prognosis. The increase expression of hTERT in colorectal adenomatous Polyp- osis may be relative with colorectal tumorigenesis.
