摘 要:目的应用蛋白芯片技术检测幽门螺杆菌(Hp)感染分布情况及其与胃十二指肠疾病的关系。方法用蛋白芯片技术检测806例有上消化道症状的患者血清中细胞毒素相关蛋白(CagA)、尿素酶(Ure)和空泡细胞毒素(VacA),对其中357例患者进一步行胃镜检查,证实其患有不同程度的胃十二指肠疾病。结果 806例患者利用蛋白芯片技术对Hp检测,无感染者40.57%,感染者59.43%,感染者中I型占7.32%;357例进行胃镜检查,I型Hp感染者主要见于慢性萎缩性胃炎(7.88%)、十二指肠球部溃疡(14.29%)、胃癌(12.50%),与其他胃十二指肠疾病比较,差异有统计学意义(P<0.05);II型、中间型、其它型Hp感染者,在各种胃十二指肠疾病中分布差异无统计学意义;无Hp感染的胃十二指肠疾病中,胃溃疡占51.52%,与其它胃十二指肠疾病比较,差异有统计学意义(P<0.05)。结论蛋白芯片技术检测Hp感染分型与胃十二指肠疾病发病情况有关系,有助于指导临床判断Hp感染者是否需根除Hp治疗。Objective To detect the distribution of Helicobacter pylori (Hp) infection and its relationship with gastropathy. Methods The distribution of serum CagA, Ure and VacA antibodies in 806 patients with upper gastrointestinal symptoms were detected using the protein chip,357 of all patients confirmed by endoscopy. Results There were 40.57% patients without Hp infection, and the remaining 59.43% patients had different degrees of Hp infection. There were 7.32% type I of Hp infection;Type I of Hp infection was found in atrophic gastritis (7.88%), duodenal ulcer ( 14.29% )and gastric cancer ( 12.50% ), which had statistics difference compared with other gastric disease (P 〈 0.05). Type II,type middle and other type Hp infection had no statistics difference in gastric disease. In gastric disease with no Hp infection, there are 51.52% of gastric ulcer, which had statistics difference compared with other gastric disease (P 〈 0.05). Conclusion Hp protein chip is facilitated for classifying the type of Hp and determining to eradicate Hp.
