Pancherz分析法评价两种功能矫治器治疗安氏Ⅱ~1类错牙合患者的效果

摘　　要：目的应用Pancherz分析法评价Dynamax、固定斜面导板治疗生长发育期Ⅱ1类错袷的临床效果．方法应用Dynamax矫治器（D组）、固定斜面导板（G组）联合固定矫治器分别对15例安氏Ⅱ1类错袷患者进行功能性矫治8一12个月，治疗前后摄头颅侧位片，应用Pancherz法进行头影测量分析，两组结果比较采用t检验。结果两组下颌骨均显著生长，治疗前后Pg/OLP、ii／OLP、mi／OLP、is／OLP．ii／OLP、ms／OLP—mi／OLP均有明显改变。前牙覆盖明显减小，D组骨性因素占52．3％，牙性因素占47．7％。G组骨性因素占34．9％，牙性因素占65．1％。磨牙关系明显改善，D组骨性因素占93．9％，牙性因素占6．1％。G组骨性因素占74．4％，牙性因素占25．6％。结论两种功能矫治器矫治Ⅱ1类错袷均可促进下颌骨生长，改善覆盖和磨牙关系。应用Pancherz法分析前牙覆盖及磨牙关系，Dynamax矫治器表现出相对较多的骨性变化。Objective To evaluate the effects between Dynamax functional appliance and Fixed Inclined Bite Plate in the treatment of Class II , division 1 malocclusion by using Pancherz＇ s analysis. Methods Thirty ehihlren with Class II , division 1 malocclusion were selected and divided into two groups averagely treated with Dynamax flmetional appli- ances （group D） and Fixed Inclined Bite Plate （group G） for total 8 - 12 months. Cephalometric radiographs wel~ takenand analyzed before and after the treatment. Students＇ t-test was used to determine if there were significant differences between the 2 time periods and between the two groups. Results Mandibles of the two groups obviously developed and Pg/OLP, ii/OLP, mi/OLP, is/OLP-ii/OLP, ms/OLP-mi/OLP had significant differences between the two groups. Overjet of the incisors were improved. In group D 52. 3% of these changes were caused by skeleton and 47.7% by teeth. In group G 34.9% of these changes were caused by skeleton and 65.1% by teeth. Molar-relationship were im- proved. In group D 93.9% of these changes were caused by skeleton and 6.1% by teeth. In group G 74.4% of these changes were caused by skeleton and 25.6% % by teeth. Conclusion Both the two appliances can effectively simulate the mandibular growth, improve the overjet and molar-relationship. The Dynamax is more effective in the skeletal changes than tile Fixed Incfined Bite Plate.
