脑梗死患者血清C反应蛋白、神经肽Y和皮质醇的动态变化及其临床意义

摘　　要：目的探讨脑梗死患者血清C反应蛋白(CRP)、神经肽Y(NPY)和皮质醇的动态变化及其临床意义。方法检测60例脑梗死患者(A组)入院第1、7及14天的血清CRP、NPY和皮质醇水平,记录神经功能缺损评分(NIHSS)。对照组为正常健康体检者33名(B组)。结果除住院第14天A组血清NPY水平与B组相仿外,A组住院期间的血清CRP、NPY和皮质醇水平均高于B组(P<0.01)。A组血清CRP和NPY在第7天最高(P<0.01)。A组入院第1天皮质醇水平最高,后逐渐降低,但第14天仍高于B组(P<0.01)。A组患者入院第1天的NIHSS评分与CRP、NPY和皮质醇呈正相关(P<0.05),第7和14天NIHSS评分与CRP和NPY呈正相关(P<0.01),但与皮质醇无明显相关性(P>0.05)。结论脑梗死患者血清CRP、NPY和皮质醇水平明显增高;炎症反应和缺血是影响病情发展的主要机制之一。Objective To study dynamic changes and significance of serum C-reactive protein （CRP） ,neuropeptide Y（NPY） and cortisol in the patients with cerebral infarction. Methods Serum levels of CRP, NPY and cortisol were measured in 60 patients with cerebral infaction on the 1st, 7th and 14th day after admission. The neurological deficits were scored by National Institutes of Health Stroke Seale（NIHSS） at same times. Thirty-three healthy people were taken as the controls. Results Except that serum level of NPY on the 14th day in group A was similar to that in group B, serum levels of CRP, NPY and cortisol of group A during hospital stay were all higher than those in group B （P〈0. 01）. Serum levels of CRP and NPY in group A were the highest on the 7th day. Serum level of cortisol was the highest on the 1st day, which on the 14th day was still higher than that in group B （P〈0. 01）. Serum levels of CRP,NPY and cortisol were positively correlated with NIHSS scores on the 1st day in group A（P〈0. 05）. Serum levels of CRP and NPY, but not cortisol, were positively correlated with NIHSS scores on the 7th and 14th day in group A（P〈0. 01）. Conclusion Serum levels of CRP, NPY and cortisol are significantly increased in the patients with cerebral infarction, for which inflammatory reaction and oxygen deficiency might be the important mechanisms.
