多层螺旋CT低张充气增强扫描诊断早期结肠癌的研究

摘　　要：目的:评价多层螺旋CT(MSCT)充气增强扫描在结肠癌诊断中的应用价值。方法:对50例经手术病理证实的结肠癌MSCT图像进行回顾性分析,以局限性肠壁增厚为观察对象,结合平扫、增强检查观察肠壁的强化方式及程度,同时采用多平面重建技术观察病变部位(MPR)。结果:50例结肠癌在平扫CT上表现为肠壁局限性增厚,增强后病变部位明显强化,MPR可以准确显示病变部位及累及范围,与术中定位一致。以病理学为金标准,MSCT充气增强扫描准确诊断结肠癌47例,其敏感性、特异性及准确性分别为96%,92%,94%。结论:MSCT充气增强扫描可以清晰显示肿块部位、大小及形态,同时联合应用MPR重建显示周围浸润、直肠受侵及淋巴结转移情况,对结肠癌准确诊断具有重要价值。Objective ：To evaluate the application of multi -slice spiral CT（MSCT） combining infation enhancement scanin the diagnosis of colon cancer. Methods ： CT images of 50 cases of pathologically confirmed colorected cancer were retrospec- tively analyzed. To observe the limited thickening of colon wall. By combining the plain scan and enhancement check. TO meas- are the extent and pattern of thickening of colon wall and executed multi - planar reconstruction （MPR） to the site of lesions. Results：The CT images of the 50 cases showed limited thickening of colon wall and the enhanced site of the lesions had been stengthened significantly. MPR can exactly show the site and the range of involsion,in accordance with the surgeon. Regarding pathology as golden norms, the sensitivity specialty and accuracy respectively 96% ,92% ,94%. during the diagnosis colon cancer of MSCT inflation enhancement scan. Conclusion：It is very valuable to conherently apply MSCT imaging and MPR to show imaging in the diagnosis of colon cancer about tumour site,size ,form, the condition of invasion rectum and lymphatic me- tastasis
