局部晚期宫颈癌MMP2、MMP9、CathepsinD、E-cadherin表达与临床预后关系的研究

摘　　要：目的:探讨局部晚期宫颈癌中MMP2、MMP9、CathepsinD、E-cadherin表达与临床病理特征及其预后的关系。方法:应用免疫组织化学法检测65例局部晚期宫颈癌中MMP2、MMP9、CathepsinD、E-cadherin表达,并分析其与临床参数和预后之间的关系。结果:MMP2的表达与组织学分型、淋巴结转移、局部复发有关(P<0.05),与UICC分期无关(P>0.05)。MMP9表达与淋巴结转移、局部复发有关(P<0.05),与组织学分型、UICC分期无关(P>0.05);CathepsinD表达与组织学分型、淋巴结转移、局部复发、UICC分期均存在相关性(P<0.05)。E-cadherin表达与淋巴结转移、UICC分期有关(P<0.05),与局部复发、组织分型无关(P>0.05);经多因素Cox比例风险模型分析显示:组织学分型、CathepsinD、MMP2的表达与局部晚期宫颈癌患者术后生存率有关(P<0.05)。结论:MMP2、MMP9、CathepsinD、E-cadherin参与了局部晚期宫颈癌的局部浸润和远处转移。组织学分型、MMP2、CathepsinD的表达可作为宫颈癌患者独立的预后因素。Objective：To investigate the relationship between cervical MMP2,MMPg,CathepsinD,E - cadherin expression and clinic pathological characteristics in locally advanced cerrical cancer. Methods：Using immunohisto- chemistry to detect the expression of MMP2, MMP9, CathepsinD,E - cadherin in 65 cases of locally advanced cervical cancer and analyze its relationship with clinical parameters and prognosis relationships. Results：There was relation- ship between MMP2＇s expression and histological type,lymph node metastasis,local recurrence （P 〈 0.05） , and no re- lationship with UICC staging（P 〉 0.05）. There was relationship between MMP9 expression and lymph node metasta- sis,local recurrence （P 〈 0.05） , and no relationship with histological type, UICC staging （P 〉 0.05 ） ; There are rela- tionship between CathepsinD expression and histological type,lymph node metastasis,local recurrence （P 〈 0.05）. There was relationship between E - cadherin expression and lymph node metastasis, UICC staging related （P 〈 0.05） , without local recurrence, histological classification （P 〉 0.05）. The result of multivariate cox proportional hazards model analysis showed that there was relationship between histological type,CathepsinD,MMP2 express and survival in patients with locally advanced cervical cancer after operation （P 〈 0.05 ）. Conclualon： MMP2, MMP9, C athepsinD, E - cadherin were all involved in locally advanced cervical cancer local invasion and distant metastasis ; histological type, MMP2,CathepsinD expression can be used as an independent prognostic factor in patients with cervical cancer.
