摘  要:目的:研究原癌基因(c-Myc)和环氧合酶-2(COX-2)在35岁以下宫颈上皮内瘤样病变(CIN)和宫颈鳞癌(SCC)患者中的表达及与微血管密度(MVD)的相关性,探讨c-Myc和COX-2在SCC发生发展中的关系及与临床病理关系。方法:选择我院手术切除的SCC组织标本40例、CIN标本31例、正常宫颈组织(NCE)10例,采用免疫组化SP法检测各组c-Myc和COX-2的表达情况及微血管密度(MVD)(CD34标记)。并通过统计学方法分析其表达情况及与肿瘤分期、病理分型、淋巴结转移等之间的相互关系。结果:c-Myc在年轻SCC中的阳性表达率为62.5%(25/40),显著高于CIN组和NCE组(P<0.01),c-Myc在年轻SCC中的表达与分化及淋巴结转移有关(P<0.01),但与分期无关。年轻SCC组中COX-2阳性表达率为67.5%(27/40),显著高于CIN组和NCE组(P<0.01),COX-2的表达与SCC的分期及淋巴结转移有相关性(P<0.01)。相关性分析发现两者在SCC组织中表达呈正相关,两者在宫颈癌血管生成中可能具有协同作用。结论:c-Myc和COX-2在年轻SCC发生、发展中起重要作用,c-Myc和COX-2上调表达可作为SCC预后的判断指标。Objectlve:To determine the expression of COX -2 and c - Myc in cervix neoplasms,CiN and SCC a- mong young women under 35 and the research of MVD,to discuss the relationship between COX - 2 and c - Myc in SCC. Methods:To detect the expressions of COX -2,c - Myc and micro vessel density(MVD) labeled by CIN3 in 40 cases in SCC,31 cases of CIN and 10 cases of NCE by immunohistochemistt7 S - P. And analyze the relationship with histological grade, pathological type,axillary lymph node metastasis. Results:The positive expression rate of c - Myc in SCC was 62.5% (25/40) which was significantly higher than that in CIN and NCE(P 〈0.01) ,the expression of c - Myc was located with myometrial invasion and lymph node metastasis, but it was unrelated with the stage (P 〈 0.01). The expression rate of COX - 2 in SCC was 67.5% (27/40) ,which was significantly higher than that in CIN and NCE (P 〈 0.01 ) ,the expression of COX - 2 was located with the stage of SCC and lymph node metastasis (P 〈 0.01 ). Expressions of them were positively correlated in SCC. Conclusion: COX - 2 and c - Myc in SCC play an important role in SCC,the high expressions of COX- 2 and c- Myc were meaningful.
