基质金属蛋白酶9表达与乳腺癌新辅助化疗疗效的关系

摘　　要：目的:探讨浸润性乳腺导管癌新辅助化疗前后血清基质金属蛋白酶9(MMP-9)水平变化及其对疗效的预测价值。方法:用双抗体夹心酶联免疫吸附法研究92例Ⅱ-Ⅲ期女性原发性浸润性乳腺导管癌行新辅助化疗前后血清MMP-9水平,并进一步探讨血清MMP-9水平与化疗疗效、化疗前临床分期的关系。结果:新辅助化疗总有效率为72.8%(67/92)。化疗前临床分期II期患者MMP-9水平低于III期患者[(952.58±463.32)U/L vs.(1286.55±527.16)U/L,P<0.05]。在新辅助化疗有效组,化疗后MMP-9水平明显降低[(1259.85±478.69)U/L vs.(501.87±398.85)U/L,P<0.01],在新辅助化疗稳定组和进展组,化疗前后MMP-9水平无明显统计学差异(P>0.05)。结论:化疗前血清MMP-9水平与化疗前临床分期存在相关性,新辅助化疗前后MMP-9水平变化与化疗疗效存在相关性,对新辅助化疗疗效预测有一定的临床意义。Objective：To investigate the significance of matrix metalloProteinase 9 in invasive ductal breast canc-er,and discuss its relation with the efficacy of neoadjuvant chemotheraPy in breast cancer. Methods：PreoPerative ser-um level of MMP - 9 was measured in 92 female Patients with stage Ⅱ - Ⅲ invasive ductal breast cancer by enzyme linked immunosorbent assay（ELISA）. We examined the association between serum level of MMP - 9 and the effi-cacy of neoadjuvant chemotheraPy,TNM stage. Results：The clinical resPonse rate of neoadjuvant chemotheraPy was 72. 8%（67 / 92）.The MMP -9 level before neoadjuvant chemotheraPy in stage II breast cancer was lower than in stage III [（952. 58 ± 463. 32）U/ L vs.（1286. 55 ± 527. 16）U/ L,P ﹤ 0. 05]. In the grouP of clinical resPonse ,the MMP -9 level after chemotheraPy was significantly lower than before chemotheraPy[（1259. 85 ± 478. 69）U/ L vs.（501. 87 ± 398. 85）U/ L,P ﹤ 0. 01]. There was no significant change in MMP - 9 level in the grouP of stable disease（SD） and Progressive disease（PD）after treated with neoadjuvant chemotheraPy（P ﹥ 0. 05）. Conclusion：The serum level of MMP - 9 was associated with the TNM stage of breast cancer. And the decreased level of MMP - 9 has clinical sig-nificance in Predicting the efficacy of neoadjuvant chemotheraPy in breast cancer.
