摘 要:目的:探讨18F-FDG PET/CT利尿延迟显像在前列腺良、恶性病变鉴别诊断中的作用。方法:选取前列腺病变患者55例,均行18F-FDG PET/CT早期及利尿延迟显像。测量病灶处两次显像最高标准摄取值(SUVmax)。55例患者均经手术或针吸法取得病理结果,并与PET/CT结果相比较。结果:病理证实前列腺癌16例,良性病变39例,良、恶性病变间的SUVmax差异具有统计学意义。18F-FDG PET/CT利尿延迟显像诊断前列腺癌的灵敏度为68.8%(11/16),特异性为89.7%(35/39),准确度为83.6%(46/55)。结论:18FFDG PET/CT利尿延迟显像是一种诊断前列腺癌较好的无创性检查方法,病灶处SUVmax可以为区别前列腺良、恶性病变提供依据。Objective:To investigate the Potential value of delayed diuretic 18 F - FDG PET/ CT scan for differenti-ating malignant from benign Prostate hyPerPlasia. Methods:A total of 55 Patients with Prostate lesions underwent 18 F- FDG PET/ CT early - delayed imagings before transrectal needly bioPsy. After attenuation correction and iterative reconstruction,two PET data were analyzed semi - quantiatively by measuring maximum standardized uPtake values (SUVmax)in Prostate lesions. Results:16 cases of Prostate cancer and 39 cases of benign Prostate hyPerPlasis or chronic Prostatitis were confirmed by Pathological diagnosis. The statistical differences of benign and malignant lesions in SUVmax were significant. The sensitivity,sPecificity and accuracy of delayed diuretic 18 F - FDG PET/ CT scan were 68. 8%(11 / 16),89. 7%(35 / 39)and 83. 6%(46 / 55)resPectively. Conclusion:Delayed diuretic 18 F - FDG PET/CT scan is a valuable non - invasive technology in the diagnosis of Prostate cancer. The SUVmax of Prostate lesions on delayed diuretic 18 F - FDG PET/ CT scan can imProve the diaglosis between benign and malignant.
