EB病毒潜伏膜蛋白1在慢性萎缩性胃炎伴肠上皮化生胃黏膜中的表达及意义

摘　　要：目的检测EB病毒（EBV）．潜伏膜蛋白1（LMPl）在作为癌前病变的慢性萎缩性胃炎（CAG）伴肠上皮化生胃黏膜中的表达，探讨其在胃癌发生、发展过程中的作用。方法应用免疫组织化学sP法检测经病理确诊的45例慢性浅表性胃炎（CSG）组织、63例CAG伴肠上皮化生组织、36例胃癌组织中EBV—LMP1的表达。结果45例CSG和36例胃癌组织中未见EBV-LMP1的表达，63例CAG伴肠上皮化生胃黏膜组织有23例阳性，阳性率为36．5％（23／63），主要定位于细胞核，明显高于CSG及胃癌中的表达，差异有统计学意义（尸值均为0．000）。结论CAG伴肠上皮化生组织中存在EBV-LMP1表达，明显高于CSG及胃癌组织，提示EBV感染可能在胃癌发生的早期阶段起重要作用。Objective To investigate the expression of Epstein-Barr virus （EBV）- latent membrane protein 1 （LMP1） in chronic atrophic gastritis （CAG） with intestinal metaplasia and discuss its effect on gastric cancer. Methods Immunohistochemistry was used to examine the expression of EBV-LMP1 in 45 cases of chronic superficial gastritis （CSG）, 63 cases of CAG with intestinal metaplasia and 36 eases of gastric cancer. Results There was no expression of EBV-LMP1 in CSG and gastric cancer, while the positive rate of EBV-LMP1 in CAG with intestinal metaplasia was 36.5%（23/63） and EBV-LMP1 was mainly stained in the cell nucleus. The expression of EBV-LMP1 in CAG with intestinal metaplasia was significantly higher than that in CSG and gastric cancer, and there was significant difference （P = 0.000）. Conclusions EBV-LMP1 is expressed in CAG with intestinal metaplasia. The expression of EBV-LMP1 is significantly higher than that in CSG and gastric cancer indicating that EBV infection in gastric carcinogenesis may play an important role in the early stages.
