参麦注射液联合生长抑素对急性胰腺炎患者血清TNF-α、IL-6及IL-10的影响

摘　　要：目的 探讨分析参麦注射液联合生长抑素对急性胰腺炎患者血清肿瘤坏死因子-α（TNF—α）、白细胞介素-6（IL-6）及白细胞介素-10（IL-10）的影响。方法 选择2011年1月～2013年9月在衢州市人民医院接受治疗的重症急性胰腺炎患者90例，随机分成两组。对照组45例患者单纯给予西药治疗。观察组45例患者在对照组治疗的基础上．增加参麦注射液及生长抑素治疗。观察两组8d内血淀粉酶以及白细胞达到正常范围的时间、腹痛症状改善与住院时间。结果 观察组总有效率（95．6％）高于对照组（80．0％），差异有统计学意义（P〈0．05）。观察组患者血淀粉酶以及白细胞达到正常范围的时间、腹痛症状改善时间与住院天数方面显著短于对照组，差异有高度统计学意义（P〈0．01）。治疗后，观察组血清TNF-α、IL-6水平为（168．66±24．64）、（15．45±5．36）ng／L，均低于对照组血清TNF-α、IL-6水平[（206．32±20．49）、（21．28±7．31）ng／L]，差异有高度统计学意义（P〈0．01）。治疗后，观察组血清IL-10水平为（99．54±16．11）ng／L，高于对照组血清IL-10水平[（69．42±15．57）ng／L]，差异有高度统计学意义（P〈0．01）。结论 参麦注射液联合生长抑素治疗重症急性胰腺炎效果显著，可以明显改善患者血清TNF-α、IL-6及IL-10水平。Objective To investigate and analyze the effects of Shenmai Injection combined with somatostatin on serum levels of tumor necrosis factor alpha （TNF-α）, interleukin-6 （IL-6） and interleukin-10 （IL-10） in patients with acute pancreatitis. Methods 90 cases of patients with severe acute pancreatitis treated in the People＇s Hospital of Quzhou City from January 2011 to September 2013 were randomly divided into two groups. 45 patients in the control group were treated with western medicine. 45 patients in the observation group were added with Shenmai Injection and somatostatin treatment on basis of control group. The time of serum amylase and white blood cells to normal range, time of abdominal pain symptom improvement and days of hospitalization were observed. Results The total efficiency of the observation group was 95.6%, which was higher than 80.0% of control group, the difference was statistically significant （P 〈 0.05）. The time of serum amylase and white blood ceils to normal range, time of abdominal pain symptom improvement and days of hospitalization in observation group were significantly shorter than those in the control group, the differences between the two groups were statistically significant （P 〈 0.01）. After treatment, the serum TNF-α, IL-6 level of the observation group were （168.66±24.64）, （15.45±5.36） ng/L, which were lower than those of the control group [（206.32±20.49）, （21.28±7.31） ng/L], the differences were ystatistically significant （P 〈 0.01）. After treatment, the serum level of IL-10 in the observation group was （99.54±16.11） ng/L, which was higher than that of the control group [（69.42±15.57） ng/L], the difference was statistically significant （P 〈 0.01）. Conclusion Shenmai Injection combined with somatostatin in treatment of severe acute pancreatitis has good curative effect, which can significantly improve the serum levels of TNF-α, IL-6 and IL-10.
