肿痛安治疗癌症化疗后口腔黏膜炎的临床应用研究

摘　　要：目的 观察肿痛安治疗癌症化疗后口腔黏膜炎的临床效果与不良反应。方法 选取成都市第三人民医院2012年1月-2013年6月癌症化疗后出现口腔黏膜炎患者236例，分为试验组80例，阳性对照组79例，阴性对照组77例。试验组给予肿痛安甘油糊剂局部治疗，阳性对照组给予地塞米松甘油糊剂局部治疗，阴性对照组给予甘油局部治疗。记录各组患者的平均溃疡期，并比较治疗前后疼痛指数变化，同时观察药物不良反应。结果 ①治疗后，试验组平均溃疡期为（4．82±1．26）d；阳性对照组平均溃疡期为（5．16±1．79）d，阴性对照组平均溃疡期为（9．57±1．64）d。试验组与阳性对照组平均溃疡期均明显短于阴性对照组，差异有统计学意义（P〈0．05）；试验组与阳性对照组间平均溃疡期比较差异无统计学意义（P〉0．05）。②治疗后试验组疼痛指数为（2．23±0．66）分；阳性对照组疼痛指数为（6．36±0．57）分；阴性对照组疼痛指数为（7．29±0．83）分。试验组疼痛指数明显低于阳性对照组与阴性对照组．差异有统计学意义（P〈0．05）。结论 肿痛安局部治疗化疗后口腔黏膜炎效果确切，相对于糖皮质激素．肿痛安的止痛效果好，而且药物不良反应相对少，安全系数更高，具有良好的临床应用价值。Objective To investigate the therapeutic effect of Zhongtong＇an in treatment of oral mucositis after chemotherapy. Methods 236 patients with diagnosis of oral mucositis after chemotherapy in the Third People＇s Hospital of Chengdu City from January 2012 to June 2013 were selected and divided into experiment group with 80 cases, positive control group with 79 cases, negative control group with 77 cases. The experiment group was treated with Zhongtong＇an-glycerol Paste, the positive control group was treated with Dexamethasone-glycerol Paste and the negative control group was treated with Glycerol. The duration of oral mucositis of the 3 groups were observed, the pain index before and after the treatment was compared. The adverse drug reactions were observed and recorded. Results （1）Duration of oral mucositis in the experiment group was （4.82±1.26） d, the positive control group was （5.16±1.79） d, and the negative control group was （9.57±1.64） d. Both experiment group and positive control group were shorter than that in the negative control group, the difference was statistically significant （P 〈 0.05）. There was no difference between experimental group and positive control group in the duration of oral mucositis （P 〉 0.05）.（2）The pain index of the experiment group was （2.23±0.66） scores, the positive control group was （6.36±0.57） scores, and the negative control group was （7.29±0.83） scores, the differences of pain index between the experiment group and the positive, negative control group were statistically significant （P 〈 0.05）. Conclusion The effect of Zhongtong＇an-glycerol Paste in the treatment of oral mucositis after chemotherapy is confirmed. Comparing with Glucocorticoid, the acesodyne effect of Zhongtong＇an is better and the adverse drug reaction is less, it is valuable for oral mucositis after chemotherapy.
