摘 要:目的观察八珍汤加减方防治乳腺癌术后创面不愈的临床疗效。方法将82例乳腺癌术后患者随机分为治疗组与对照组,每组41例。两组均先予常规围手术期抗生素治疗,治疗组加用八珍汤加减方,对照组加用维生素C静脉滴注。两组疗程均为2周,观察创面不愈事件发生率(局部血运差、皮瓣坏死、皮下积液、创面感染、上肢水肿)及引流管放置时间,并观察超敏CRP水平的变化情况。结果①治疗组、对照组创面不愈事件的总发生率分别为12.2%、90.2%;两组局部血运差、皮瓣坏死、皮下积液、创面感染及上肢水肿的发生率差异有统计学意义(P<0.05)。②两组引流管放置时间差异有统计学意义(P<0.05)。③治疗前后组内比较,两组超敏CRP水平差异均有统计学意义(P<0.05);组间治疗后比较,超敏CRP水平差异有统计学意义(P<0.05)。结论八珍汤加减方可明显促进乳腺癌术后创面修复,减少创面不愈的发生,其机制可能与改善局部血供及减少炎症反应有关。Objective To observe the clinical efficacy of modified " Bazben Decoction" on wound healing in postoperative patients with breast cancer. Methods Eighty-two cases were randomized into treatment group and control group, with 41 cases in each group. Both of the groups were treated by conventional antibiotic therapy. Additionally, treatment group was treated with modified " Bazhen Decoction" orally and control group was with Vitamin C injection, with the course of 2 weeks. The occurrence rate of events and staying time of drainage tube were observed after treatment, and the CRP level was observed before and after treatment. Results (1) After treatment, the occurrence rate of events was 12. ?% in treatment group and 90.2% in control group; there were significant differences in blood supply, flap necrosis, subcutaneous effusion, wound infection and upper extremity edema between the two groups ( P 〈 0.05 ). (2) After treatment, there was a significant difference in staying time of drainage tube between the two groups ( P 〈 0.05 ). (3) In each group, significant difference was found in CRP level between before and after treatment ( P 〈 0.05 ). After treatment, there was a significant difference in CRP level between the two groups ( P 〈 0.05 ) ; Conclusion Modified " Bazhen Decoction" can promote the wound healing in postoperative patients with breast cancer. Its mechanism may be association with improving local blood supply and reducing inflammatory reaction.
