食管癌术后吻合口瘘两种治疗方法对比观察

摘　　要：目的探讨食管癌术后吻合口瘘24例的治疗。方法对24例食管癌术后吻合口瘘进行回顾性分析,其中传统方法治疗吻合口瘘14例,给予禁食、保持胸腔引流管通畅及营养支持;生理盐水持续冲洗瘘口方法治疗吻合口瘘10例,给予禁食、保持胸腔引流管通畅及营养支持同时,给予生理盐水持续冲洗瘘口。结果第一阶段14例吻合口瘘中有6例死亡,病死率为42%;第二阶段吻合口瘘10例,无一例发生死亡。结论生理盐水持续冲洗瘘口方法可提高食管癌吻合口瘘的治愈率。Objective To study the therapeutic of 24 cases of anastomotic leakage in post-esophagogastrectomy.Methods 24 cases of esophageal anastomotic fistula were retrospectively analyzed.14 cases of anastomotic leak were treated by the traditional methods which fasting,keeping chest tube patency and nutritional support was given.10 cases of anastomotic fis-tula were treated with the 0.9%NaCl continuous irrigation method which fasting,keeping chest tube patency and nutritional support was also given at the same time.Results In the first phase 6 of 14 cases of anastomotic fistula died,mortality rate was 42%.In the second phase,no deaths occurred in 10 cases of anastomotic leakage.Conclusion 0.9% NaCl fistula continuous irrigation method can improve the cure rate of esophageal anastomotic leak in esophagus cancer patients.
