摘 要:目的探讨Ki-67、HER-2和P53在乳腺癌组织中的表达及其与临床病理特征的相关性。方法收集2010年1月~2014年1月皖南医学院第一附属医院收治的乳腺癌患者共215例。统计分析Ki-67、HER-2和P53的表达与临床病理特征之间的相关性。结果 Ki-67的表达与肿瘤体积(χ2=10.88,P=0.012)、病理类型(χ2=6.67,P=0.010)、组织学分级(χ2=14.07,P=0.001)显著相关,而与年龄、绝经状态、淋巴结转移无明显相关性(P>0.05);其表达与雌激素受体(r=-0.281,P=0.002)、孕激素受体(r=-0.224,P=0.007)的表达呈负相关,而与P53(r=0.209,P=0.021)、HER-2(r=0.225,P=0.013)的表达呈正相关。HER-2的表达与年龄、绝经状态、病理类型、肿瘤体积、淋巴结转移、组织学分级均无明显相关性(P>0.05);其表达与雌激素受体表达呈负相关(r=-0.213,P=0.018),而与孕激素受体、P53的表达无明显相关性。P53的表达仅与组织分级(χ2=7.57,P=0.023)相关,而与年龄、绝经状态、病理类型、淋巴结转移均无明显相关性(P>0.05);其表达与雌激素受体的表达(r=-0.254,P=0.005)呈负相关,而与孕激素受体的表达无相关性。结论 Ki-67、HER-2和P53的表达与患者的临床病理特征存在内在的相关性,其对综合判断乳腺癌的恶性程度具有重要的临床意义。Objective To investigate the association of clincopathological features with the expression of Ki- 67,HER-2, and P53 in breast carcinoma patients. Methods 215 patients were selected from patients with breast cancer from January 2010 to January 2014 at the First Affiliated Hospital of Wannan Medical College. Statistical analysis was performed to determine the relationship between the expression of Ki-67, HER-2, P53 and the clinicopathological features of breast cancer patients. Results The Ki-67 expression was associated significantly with tumor size (χ2=10.88,P=0.012), pathology (χ2=6.67,P=0.010), histological grade (χ2=14.07,P= 0. 001). However, there was no statistically signicant difference in the expression of Ki-67 with age, menopausal status, lymph node metastasis (all P〉0.05). The expression level of Ki-67 was negatively correlated with estrogen receptor (ER) expression ( r =-0. 281, P 〈 0. 002) and progesterone receptor (PR) expression ( r =-0. 224, P = 0. 007), and positively with HER-2 expression (r=0. 209,P=0. 021) and P53 expression(r=0. 225,P=0. 013). The HER-2 expression was not statistically different in age, menopausal status, tumor size, pathology,histological grade, lymph node metastasis ( all P 〉 0. 05 ), it was negatively correlated with ER expression ( r =-0. 213, P = 0. 018) ,and was not correlated with PR and p53 expression. The expression of P53 was only associated significantly with histological grade ( χ2 = 7.57, P= 0. 023), but not with age, menopausal status, tumour size, pathology,lymph node metastasis (all P^0.05), and was not correlated with PR expression. It was negatively correlated with ER expression (r=-0. 254, P= 0. 005), but not correlated with PR expression. Conclusion Our data suggest that the expression of Ki-67, HER-2 and P53 were correlated with the clincopathological features in breast carcinoma patients.
