高危型HPV感染宫颈病变治疗研究

摘　　要：目的 通过使用高能聚焦超声和利普刀治疗高危型人乳头状瘤病毒（HPV）感染宫颈病变,比较其疗效和安全性,寻找治疗高危型HPV感染宫颈病变的有效措施.方法 将2010年5月～2012年1月确诊感染高危型HPV宫颈病变90例患者,随机分为聚焦超声治疗组、利普刀治疗组及对照组,随访并评价3组患者的HPV-DNA滴度变化和治疗效果.结果 治疗后6个月和12个月超声组、利普刀组与对照组比较,HPV降低时间短、转阴率高,差异有统计学意义（P＜0.05）.超声组和利普刀组治愈率比较,差异无统计学意义（P＞0.05）.超声组、利普刀组与对照组治疗后6个月和12个月治愈率比较,差异有统计学意义（P＜0.05）.利普刀组2例复发,而超声组1例复发.结论 高能聚焦超声是治疗高危型HPV感染宫颈病变有效的方法,可用于临床治疗.Objective To compare the effectiveness and safety of focused ultrasound （FU） verses Loop Electrosurgieal Excision Procedure （LEEP） in the treatment of the cervical disease with high-risk human papillomavirus （HPV）infection. Methods 90 cases of the cervical disease with high-risk human papillomavirus infection between May 2010 and January 2012 were followed-up and evaluated the patient outcomes and HPV-DNA changes with the treatment by FU, by LEEP and the control groups. Results After 6 months and 12 months of treatment,the efficacy of the FU group or the LEEP group was significantly better than the control group （P〈 0.05）, and the efficacy of the FU group was not significantly different from the LEEP group （P〉0.05）. In the LEEP group 2 patients relapsed, while in FU group 1 patients relapsed. The positive values of HPV in FU group were decreased more than the LEEP group, with the significant difference （P〈 0. 05）. Conclusion Foused ultrasonic therapy is a kind of safe and effective method for the treatment of the cervical disease with high-risk human papillomavirus infection.
