摘　　要：目的从卫生经济学角度对湖南省长沙地区农村妇女进行子宫颈癌和乳腺癌筛查结果进行评价,为卫生管理者循证决策提供科学依据。方法对2009-2012年湖南省长沙地区35-59岁农村妇女＂两癌＂筛查结果进行成本效果、成本效用和成本效益分析。结果 （1）子宫颈癌和乳腺癌患病率分别为40/10万和56/10万,病死率分别为7/10万和9/10万;（2）子宫颈癌和乳腺癌筛查项目成本分别为2818.18万元和2126.91万元;（3）发现1例子宫颈癌和乳腺癌的成本分别为15.40万元和12.37万元;（4）每挽救1个失能调整生命年子宫颈癌和乳腺癌所需成本分别为4748.20元和3777.43元;（5）子宫颈癌和乳腺癌的效益成本比分别为1.64∶1和1.86∶1。结论 ＂两癌＂筛查项目成本低、效果好、效益高,可以有效控制子宫颈癌和乳腺癌,降低个人、家庭和社会的负担,应继续推动＂两癌＂筛查工作的可持续发展。Objective For the point of view of health economics cervical cancer and breast cancer screening projects carrying in regional rural women in Changsha city,Hunan Province were evaluated in order to provide a scientific basis for evidence-based decision making. Methods From 2009 to 2012 in Changsha city,Hunan Province,35 - 59 years old rural women underwent cervical cancer and breast cancer screening. Cost effect, cost effectiveness and cost-benefit effect of the screening projects were analyzed. Results（1） Incidence of cervical cancer and breast cancer were 43/100 000 and 56/100 000 respectively,corresponding mortality were 7/100 000 and 9/100 000 respectively;（2） Cervical cancer and breast cancer screening project cost 2818.18 million yuan and 2126.91 million yuan respectively;（3） Cost of detecting 1 case of cervical cancer or breast cancer was 15.40 thousand yuan and 123.7 thousand yuan respectively;（4） To save 1 disability adjusted life years, cervical cancer or breast cancer costs 4748.20 yuan and 3777.43 yuan respectively;（5） Benefit cost ratio of cervical cancer and breast cancer were 1.64 ∶ 1 and 1.86 ∶ 1 respectively. Conclusion Cervical cancer screening and breast cancer screening projects are low cost,effective and high efficacy as an effective method to control cervical cancer and breast cancer. The projects reduce the economic burden of individuals,families and society. Therefore it should continue to promote the sustainable development of the two cancer screening projects.
