摘 要:目的比较光学相干生物测量仪(IOL Master)与接触式A超测量眼轴长度(AL)的精确性,为不同屈光状态的白内障患者IOL度数的测算提供参考。方法临床病例对照研究。对2013年1~3月在天津市眼科医院就诊的准备行超声乳化白内障吸除联合后房型IOL植入术的年龄相关性白内障患眼300只眼,将其分为4组:高度远视组(AL≤22mm)60只眼;相对正常眼组(22mm〈AL≤26mm)120只眼;高度近视A组(AL〉26mm,不伴后巩膜葡萄肿)60只眼;高度近视B组(AL〉26mm,伴后巩膜葡萄肿)60只眼。分别应用IOLMaster和接触式A超测量AL。结果IOL Master和接触式A超所测AL高度远视眼组,分别是(20.58±0.43)mm、(20.51±0.52)mm,两者之间差异无统计学意义P〉0.05(t=1.36,P=0.18);相对正常眼组分别是(23.85±0.75)mm、(23.81±0.73)mm,两者对比差异无统计学意义P〉0.05(t=1.70,P=0.09);高度近视眼A组,分别是(28.27±1.11)mm、(28.25±1.00)mm,两者之间差异无统计学意义P〉0.05(t=0.40,P=0.70);高度近视眼B组,分别是(29.44±2.28)mm、(29.19±2.20)mm,两者之间的差异具有统计学意义P〈0.05(t=3.93,P=0.00),IOL Master测量AL较接触式A超长。结论对于高度远视和近视眼白内障(A组)患者,IOL Master与接触式A超测量眼轴长无明显差异,而伴有后巩膜葡萄肿的高度近视眼的眼轴测量结果应结合IOL Master和接触式A超的测量结果。Objective To compare the measurement of axial length (AL) for routine cataract eyes by Optical coherence biometry (IOL Master) and contact ultrasonic axial scan (CUSAS). Meth- ods Measurements of AL were prospectively obtained in 300 eyes of 286 subjects with the IOL Master compared with that from the CUSAS. Results The mean values of AL measured by IOL Master and CUSAS were(20.58±0.43) mm and (20.51±0.52) mm in the hyperopia group (AL≤ 22mm), (23.85±0.75) mm and (23.81±0.73) mm in the normal group (22 mm〈AL≤26mm), (28.27± 1.11) mm and (28.25±1.00) mm in the high myopia group A (AL〉26 mm without posterior scleral staphyloma), (29.44±2.28) mm and (29.19±2.20) mm in the high myopia group B (AL〉26mm with posterior scleral staphyloma). There was a statistically significant difference between IOL Master and CUSAS measurements for AL in the high myopia group B (t=3.93, P=-0.00) while there was no stati-cally significant difference in other groups (P〉0.05). Conclusions As a non-contact biometry, IOL Master provides accurate values. A high degree of agreement between IOL Master and CUSAS is noted. Further studies are needed to assess the interchangeability of measurements.
