翼状胬肉切除术中配戴角膜接触镜疗效分析

摘　　要：目的研究翼状胬肉切除术中配戴角膜接触镜对角膜创面上皮修复及术后复发率的影响。方法对2010年12月至2012年8月在佛山市三水人民医院眼科就诊的30例双眼翼状胬肉患者，其胬肉位于鼻侧，长入角膜缘内2．5mm以上。按手术顺序随机分为2组：即在行胬肉切除手术时，随机选取1只眼为A组，另1只眼为B组。A组（30只眼），胬肉切除＋丝裂霉素C＋羊膜移植；B组（30只眼），胬肉切除＋丝裂霉素C＋羊膜移植＋角膜接触镜。术后用l％荧光素钠染色法，观察角膜创面上皮修复速度，并随访6—12个月，平均8．5个月。结果A、B组患者角膜创面愈合时间分别为（2．3±1．2）d、（1．6±0．7）d，以A组较长（P〈0．05）；术后A组中1只眼巩膜变薄，经用新鲜羊膜移植后好转。2组术后复发中比较，A组3只眼复发（10％），而B组仅1只眼复发（3．3％），差异具有统计学意义（P〈0．05）。结论翼状胬肉切除术中应用角膜接触镜在缩短角膜上皮的愈合时间和降低术后复发率方面，效果较为满意。Objective To investigate the effects of pterygium excision with corneal contact lens on corneal epithelia healing and recurrence rate. Methods Thirty patients scheduled for pterygium excision surgery were recruited in this randomized, single blind and prospective study. Patients were randomly divided into Group A and Group B. Patients in Group A （30 eyes） were performed pterygi-um excision with mitomycin C and amniotic membrane transplantation and patients in Group B （30 eyes） were performed pterygium excision with mitomycin C, amniotic membrane transplantation and corneal contact lens wearing. In pterygium excision operation, one randomly selected eye was for Group A, and the other eye was Group B. A follow-up of 6 to 12 months were then performed and the corneal wound healing was observed by staining of 1% sodium fluorescein. Results One patient suffered scleral thinning in Group A and it improved after the second fresh amniotic mem-brane transplantation. The mean corneal would healing time of both groups was （2.3 ±1.2） days and （1.6± 0.7） days, respectively （P 〈0.05）. The recurrence rate of both groups was 10% （3 eyes） and 3.3% （1 eye）, respectively （P 〈0.05）. Conclusions Application of intraoperative corneal contact lens provides satisfactory effects in promoting corneal epithelial healing and reducing the postopera-tive recurrence rate of pterygium excision.
