自拟益气润目汤治疗干眼的临床研究

摘　　要：目的观察干眼经验方——益气润目汤内服联合外部熏蒸治疗干眼的临床疗效。方法将符合干眼诊断标准的113例患者,随机分为治疗组和对照组,治疗组59例(109只眼),对照组54例(104只眼)。治疗组采用益气润目汤内服联合外部熏蒸治疗干眼,对照组采用单纯局部滴用玻璃酸钠滴眼液治疗。根据主观症状和基础泪液分泌试验、泪膜破裂时间(BUT)、角膜荧光素钠染色等检查进行评分,治疗观察4周。结果2组治疗后BUT、Schirmer I试验、角膜荧光素染色评分好于治疗前,差异均有统计学意义(P<0.05),治疗组的各指标变化程度明显优于对照组(P<0.05);治疗组总有效率84.4%,对照组总有效率70.2%,2组差异有统计学意义(P<0.05)。结论干眼经验方——益气润目汤内服联合局部熏蒸能促进泪液生成、眼表损害修复,有助于维护泪膜的稳定性,改善干眼症状。OBJECTIVE To observe the clinical efficacy of Yiqi Runmu decoction combined with external fumigation treating dry eye. METHODS A randomized, parallel-control approach was adopted, 113cases of dry eye patients were divided into treatment group （59cases, 109 eyes） and control group （54 cases, 104 eyes）, and observed the treatment for four weeks. The treatment group was applied Chinese medicine combined external fumigation, simply hyaluronate sodium eye drops for control group. Before and after treatment, the tear secretion volume, break-up time, corneal fluorescein staining and symptom were observed. RESULTS After treatment, there were significant statistical differences for the break-up time, Schirmer I test and corneal fluorescein staining in both groups when compared with before treatment （P〈0.05）. Each index change was significantly better than that of control group （P〈0.05）; The total clinical effective rates were 84.4% for treatment group and 70.2% for control group, both with no serious adverse reactions. CONCLUSIONS Yiqi Runmu decoction combined with local fumigation could stimulate the tear secretion, promote ocular surface damage repairing, and maintain the stability of the tear film, improve the uncomfortable symptoms of dry eve.
