联合图形视觉诱发电位及中心视野检查在儿童弱视综合治疗评估中的意义

摘　　要：目的采用回顾性分析的方法观察经综合治疗后弱视儿童图形视觉诱发电位(PVEP)P100波及视野的变化情况,为临床寻找一种全面、客观、可靠的弱视诊疗评估系统。方法 2009至2012年就诊于陕西省人民医院并经综合治疗且随访1年的弱视儿童病例82例133只眼,所有病例按照弱视程度分为轻、中、重度3组,并设立同年龄正常对照;分别记录治疗前后PVEP P100的反应值,并观察视野变化的情况,统计数据采用SPSS 19.0统计软件处理。结果随着弱视程度加重,PVEP P100的振幅降低,潜伏期延长,中心10°视野的平均光敏感度降低;正常组、轻、中、重度弱视之间PVEP P100的潜伏期及振幅存在不同程度差异;轻度弱视10°视野的平均光敏感度(MS)与中、重度弱视组存在明显差异(P<0.01)。经综合治疗后,轻、中度弱视眼P100潜伏期缩短,振幅增加,10°视野MS增加(P<0.05),重度弱视治疗前后各指标改善不明显(P>0.05)。结论综合治疗可有效治疗儿童弱视,PVEP及中心视野是评估弱视儿童视功能的客观方法和分析指标。OBJECTIVE To observe the difference of wave P100 in pattern visual potentials （PVEP） and changes in visual field, to find a systemic method for comprehensive, objective and reliable treatment and evaluation of amblyopia. METHODS One hundred and thirty-three eyes in 82 children with amblyopia were examined, these children were examined in Shaanxi provincial peoples＇ hospital in 2009-2012. All cases were divided into mild, medium and serious groups, and with normal control grope at the same time. The differences of wave P100 in pattern visual potentials （PVEP） and retina light sensitivity of 10-degree area of the macula were recorded after amblyopia treatment. The data was analysed by SPSS 19.0. RESULTS Latent period of wave P100 prolonged, the amplitude reduced and the retina light sensitivity of 10-degree area of the macula cut down as amblyopia aggravating. There was difference of wave P100 and 10-degree vision field among normal, mild, medium and serious groups （P〈0.05）. And distinct difference was found between before and after treatment （P〈0.05）. No difference in serious group （P〉0.05）. CONCLUSIONS Comprehensive therapy for children amblyopia could achieve satisfactory effects. PVEP and central field of view were proved to be objective and effective in amblyopia assessment.
