摘  要:目的通过观察不同持续时间高浓度吸氧对大鼠肺组织NF-кB和IL-8的影响,探讨高浓度氧所致肺组织损伤(HILI)的发生时间和机制。方法 40只成年雄性Wistar大鼠按随机数字表法分为5组,即对照组(吸入空气)、吸氧48 h组、72 h组、96 h组和120 h组,吸入氧浓度均在95%以上。测定肺组织湿干重比值(W/D)和支气管肺泡灌洗液(BALF)中性粒细胞计数;采用免疫组织化学法检测肺组织NF-кB和IL-8的表达水平。结果随着高浓度氧暴露时间延长,肺组织W/D、肺组织NF-кB和IL-8蛋白表达水平及BALF中性粒细胞计数均逐渐增加,其中96 h组和120 h组增加最明显,与对照组、48 h组及72 h组比较均有统计学意义(均P<0.01),而对照组、48 h和72 h组之间,以及96 h组和120 h组之间各项指标比较差异均无统计学意义(均P>0.05)。相关性分析表明,肺组织NF-кB与IL-8蛋白表达水平之间呈正相关(r=0.856,P<0.001)。结论高浓度吸氧持续96 h以上即可引起肺组织损伤,其发生可能与高浓度氧激活肺组织细胞NF-кB,上调IL-8表达,导致中性粒细胞聚集、活化引起的炎症反应有关。Objective To explore the time and the mechanism of hyperoxia-induced lung injury(HILI) by observing the effects of NF-кB and IL-8 in the lung tissue of the wistar rats suffered from exposure to high concentration oxygen of different duration. Methods 40 wistar rats were randomly divided into control group, oxygen inhaling 48 h group, 72 h group, 96 h group and 120 h group and the inhaled-oxygen concentration was over 95%. The wet and dry weight ratio(W/D) of lung tissue and the neutrophils count in bronchoalveolar lavage fluid(BALF) were measured. The optical density(OD) of nuclear factor-кB(NF-кB) and IL-8 in lung tissue was analyzed by immunohistochemistry. Results As the exposure duration to high concentration oxygen was prolonged, the data of W/D of the lung, neutrophils count in BALF, the OD of NF-кB and IL-8 in lung tissue were gradually increased, especially in the 96 h and 120 h group which were significantly increased compared to the control group,48 h and 72 h group(P〈0.01). Of the indicators above, there were no significant difference existed among control group, 48 h and 72 h group, neither between the 96 h and the 120 h group(P〉0.05). Correlation analysis shows that NF-кB and IL-8 were positively correlated(r=0.856, P〈0.001). Conclusions The occurrence of HILI was 96 hours when exposing to high concentration oxygen, which may be associated with the activation of NF-кB. NF-кB when activated can up-regulate the expression of IL-8 in the lung tissue of rats and lead to the inflammatory injury eventually by way of aggregation and activation of neutrophils.
