急性脑梗塞患者的事件相关电位P300研究

摘　　要：目的评价事件相关电位P300对急性脑梗塞患者认知功能预后判断的临床价值。方法对80例急性脑梗塞患者,40例正常对照组采用电生理技术检测事件相关电位P300,6个月后应用简明精神状态量表(MMSE)评价认知功能。结果①脑梗塞痴呆组P300潜伏期(392.5±32.6)ms较脑梗塞非痴呆组(335.8±31.3)ms和正常对照组合(345.6±29.7)ms明显延长(P<0.01)。②脑梗塞痴呆组P300振幅(6.2±0.7)μV较脑梗塞非痴呆组(10.7±1.2)μV和正常对照组(9.8±0.8)μV明显减小(P<0.01)。③脑梗塞痴呆组MMSE评分与P300潜伏期呈负相关(r=-0.547,P<0.01)与P300振幅呈正相关(r=0.529,P<0.05);非痴组均无相关性。结论急性脑梗塞患者P300潜伏期延长、振幅减小可预测认知功能的不良预后,为并发血管性痴呆做好早期诊断和治疗。Objective To evaluate the clinical value of event related potential P 300 on cognitive function of patients with acute cerebral infarction prognosis.Methods 80 cases of acute cerebral infarction , 40 cases of normal control group with electrophysiological technique for the detection of event related potential P 300 , application of mini mental state ex-amination after 6 months （ MMSE） the evaluation of cognitive function.Results The cerebral infarct dementia latency of P300 （392.5 ±32.6）ms with cerebral infarction in non dementia group （335.8 ±31.3）ms and normal control group （345.6 ±29.7）ms was significantly prolonged （P〈0.01）.②cerebral infarction dementia group （6.2 ±0.7） P300 amplitude μV with cerebral infarction in non dementia group （10.7 ±1.2）μV and normal control group （9.8 ±0.8）μV was significantly reduced （ P〈0.01 ）.③cerebral infarction dementia group the MMSE score was negatively correlated with the latency of P300 （r=-0.547, P〈0.01） and P300 amplitude was positively （r=0.529, P〈0.05）;there was no correlation between non Chi group.Conclusions Acute cerebral infarction with P300 latency, amplitude reduction can be prolonged adverse prognostic prediction of cognitive function , to make early diagnosis and treatment for patients with vascular dementia.
