经上颌窦眶下神经撕脱联合阿霉素神经干注药治疗三叉神经Ⅱ支痛

摘　　要：目的：总结经上颌窦眶下神经撕脱，联合阿霉素神经干注药，治疗三叉神经Ⅱ支痛的疗效。方法：对32例原发性三叉神经Ⅱ支痛患者行经上颌窦眶下神经撕脱联合阿霉素神经干注药治疗，术后对25例患者复诊或电话随访5年以上。结果：31例患者在术后1周内三叉神经疼痛症状完全消失，1例无效；术后均并发眶下区不同程度肿胀1～2周，伴患侧眶下区、上唇区、上颌牙齿、鼻翼部等区域麻木；25例术后随访5年以上，共5例复发，其中板机点在眶下神经支配区的仅2例复发。结论：经上颌窦眶下神经撕脱联合阿霉素神经干注药治疗三叉神经Ⅱ支痛，对于板机点在眶下神经支配区的三叉神经Ⅱ支痛，是一种安全、简单、创伤小而非常有效的手术治疗方法。Objective： To review the effects of nerve avulsion technique combined with adriamycin nerve trunk injection in the treatment of trigeminal neuralgia （Ⅱ）. Methods： 32 patients with the trigeminal neuralgia （ Ⅱ） were comprised in this study. All patients were underwent cooperation treatment of avulsion of the infraorbital nerve through the maxillary sinus, and injection of adriamycin to the infraorbital nerve trunk. 26 patients were followed up over 5 years. Results：Trigeminal neuropathic pain in all patients were relieved within one week after operation. Among the 26 follow-up cases, 5 patients recurred. Conclusion：This therapy is a safe, simple, minimal invasive, and effective method for relieve trigeminal neuralgia（Ⅱ） which has a trigger point innervated by the infraorbital nerve.
