射频消融与肝动脉栓塞化疗对原发性肝癌的短期疗效评价

摘　　要：目的：应用倾向分值匹配法均衡射频消融组（RFA组）和肝动脉栓塞化疗组（TACE组）患者间的临床基线资料，通过匹配后两组患者的短期生存率比较评价两种治疗方法的优劣。方法收集经RFA或TCAE治疗的137例肝细胞肝癌患者的临床基线资料（其中经RFA治疗者78例，经TACE治疗者59例），采用倾向分值匹配法对两组患者进行卡钳配对，对匹配后获得的新样本进行生存率分析。结果未经匹配之前，除性别、AFP值外的所有临床基线资料的P值均〈0.05，经匹配后，新样本的所有临床基线资料的P值均〉0.05，说明这些自变量经匹配后在两组间的差异无统计学意义，两组新样本均衡可比；经匹配后，RFA组和TACE组的1年生存率分别为63.39%和54.28%，两年生存率分别为36.15%和29.08%，中位生存时间分别为1.12年和0.94年，P值0.041，差异有统计学意义。结论应用倾向分值匹配法均衡组间差异后，RFA组的短期生存率优于TACE组。Objective Applying propensity score matching method to balance the baseline clinical data in radiofrequency ablation group （RFA group） and transcatheter arterial chemoembolization group （TACE group） between patients of hepatocellular carcinoma, by matching the two groups of patients after the short-term survival rate compared to evaluate the two treatment methods. Methods The clinical baseline data of 137 cases of hepatocellular carcinoma in patients with RFA or TCAE treatment （including the RFA treated 78 cases, treated by TACE 59 cases）, using propensity score matching method for caliper pairing of two groups of patients, the new sample after matching was obtained for the survival analysis. Results Before matching, all baseline clinical information except gende, AFP values and P values were less than 0.05, after the match, all the baseline clinical data of the new sample P values were more than 0.05, the difference between the two groups by matching these variables did not have statistical significance, two groups of samples balance were comparable;After the match, the 1 year survival rate of RFA group and TACE group were 63.39%and 54.28%, and two years survival rates were 36.15%and 29.08%, the median survival time was 1.12 years and 0.94 years, the P value is 0.041, the difference was statistically significant. Conclusion The application of propensity matching score differences between groups were balanced, RFA group short-term survival rate is better than that of TACE group.
