腹腔镜与开腹直肠癌全系膜切除术淋巴结清扫效果的比较

摘　　要：目的比较腹腔镜与开腹直肠癌全系膜切除术的淋巴清扫效果。方法回顾性分析2010年10月至2013年7月间125例直肠癌根治术患者的临床资料,其中腹腔镜组63例,开腹组62例,比较两组患者淋巴结清扫情况。结果两组患者检出淋巴结总数及各站淋巴结数比较,差异无统计学意义(P>0.05)。腹腔镜组患者手术时间明显长于开腹组,但是切口长度、术中出血量、肠道排气时间及术后住院时间均明显少于开腹组,差异有统计学意义(P<0.05)。结论腹腔镜直肠癌全系膜切除术可以获得开腹手术相似的淋巴结清扫效果,但是腹腔镜手术具有微创的优势。Objective To evaluate the lymph node cleaning effect of total mesorectal resection by laparoscopic and open surgery in eolorectal cancer patients. Methods A retrospective analysis of 154 cases of patients with colorectal cancer and received radical resection of rectal carcinoma in our hospital from Oct 2010 to Jul 2013, which were divided into laparoscopie group（63 cases） and open surgery group（62 ca- ses）. The effect of the lymph node cleaning of the two groups were observed. Results There were no statistically significant difference in the total number of lymph nodes and the number of lymph node in each sta- tions（ P 〉 0. 05 ）. The index of operative time of the laparoscopic group were significantly longer than that of the traditional open surgery group, but the length of the incision, postoperative peri-operative bleeding and postoperative hospitalization intestinal recovery were significantly superior. Conclusion The lymph node cleaning effect of total mesorectal resection by laparoscopic and traditional open surgery was similar, but the laparoscopic surgery had advantages of minimally invasive.
