动脉介入治疗消化道肿瘤后腹膜淋巴结转移的临床价值

摘　　要：目的:探讨采用动脉介入治疗消化道肿瘤后腹膜淋巴结转移的临床价值。方法:选取因消化道肿瘤后腹膜淋巴结转移而住院治疗的56例患者,采用奥沙利铂和5氟尿嘧啶动脉介入的方法治疗,随访12个月观察其疗效(淋巴结的大小、缓解情况、生存率以及生活质量评价等)及不良反应发生情况。结果:术后1个月后疗效评价:淋巴结大小由术前(22.3±5.1)cm2降为术后(10.1±2.3)cm2;完全缓解6例,部分缓解42例,稳定5例,进展3例,总有效率85.7%;术后6个月、12个月生存率分别为83.9%、73.2%;患者的生活质量评价:术前61.30±4.00,术后67.50±6.00。未发生严重不良反应。结论:动脉介入治疗消化道肿瘤后腹膜淋巴结转移是安全有效的,是后腹膜淋巴结转移可以选择的理想治疗方法。当然,该方法仍需进一步的研究评估。Objective To evaluate the clinical value of artery interventional treatment for retroperitoneal lymph node metastasis from gastrointestinal tumors. Method 56 patients with retroperitoneal lymph node metastasis from gastrointestinal tumors were enrolled. And they were given oxaliplatin and 5 - fluorouraeil transeatheter arterial infusion chemotherapy. The evaluate efficacy such as the size and defuse of lymph nodes,the survival rate,quality of life score and adverse events were observed in s year. Results After one month the size of lymph nodes become from （22. 3 ± 5.1） cm2 to （ 10. 1 ± 2. 3） cm2. And there were 6 eases of complete remisslon,partial remission in 42 eases,and 5 eases of steady progress 3 cases,and the total efficiency was 85.7%. The survival rates were 83.9% in 6 months,and 73.2% in a year. The quality of life assessment beeome from （ 61.30 ± 4. 00） to （ 67.50 ± 6. 00）. Moreover, there were no serious adverse reactions. Conclusion Artery interventional treatment for retroperitoneal lymph node metastasis from gastrointestinal tumors is safe and effective, And it＇s an ideal method to control the pain of the retroperitoneal lymph node metastases. Of course, this method requires further study assessed.
