应用铰链型膝关节治疗胫骨近端骨巨细胞瘤中期疗效分析

摘　　要：目的：探讨定制国产铰链膝关节肿瘤假体治疗胫骨近端骨巨细胞瘤的临床疗效。方法：对收治的10例胫骨近端骨巨细胞瘤行瘤段切除后应用国产铰链型膝关节肿瘤假体重建患者进行回顾性研究。所有患者均为初次就诊。采用骨与软组织肿瘤协会评分（MSTS）标准评定膝关节功能。结果：10例患者均获得随访,平均随访时间41.4个月（12~80个月）。无死亡、无局部复发及远处转移病例。膝关节伸直0°,屈膝70°~100°。MSTS为（25.4±4.8）分,优良率为90%。无切口并发症及深部感染。所有病例均未发现明显假体松动、下沉。结论：胫骨近端骨巨细胞瘤行瘤段切除后应用定制国产铰链膝关节肿瘤假体重建功能,临床效果满意。Objective To investigate the clinical effects of giant cell tumor of proximal tibia treated with custom made rotating binge ar- tificial total knee replacement. Method 10 patients with giant cell tumor of proximal tibia were collected, They were treated with rotating hinge prosthesis replacement. Bone and soft tissue tumor association scoring was used to evaluate the post - operation knee func- tions. Results All tile patients were followed - up with an average time of 41.4 months（ range 12 - 80 months）. There was no recurrence or distant metastasis in all patients. ALL patients were survived. The extension and flexion of knee activity was 0° and 70° - 100°respective- ly. The average MSTS functional score is （25.4±4. 8）. The excellent and good rate was 90%. No infection and loosening or subsidenence of prosthesis was observed. Conclusion Custom made rotating hinge artificial total knee replacement is an effective limb salvage method for the treatment of giant cell tumor of proximal tibia.
