面中部脱套入路治疗鼻眶筛区骨折

摘　　要：目的评价面中部脱套入路在鼻眶筛骨折复位内固定术中的应用价值。方法 9例鼻眶筛区骨折患者,均经面中部脱套入路行骨折切开复位内固定术,总结评价该手术入路治疗鼻眶筛区骨折的适应证、并发症和疗效。结果 9例患者均成功完成骨折切开复位内固定手术,术后效果满意,面部无切口瘢痕。2例术后眶下区麻木经保守治疗恢复,1例术后出现鼻前庭环状瘢痕性狭窄,经手术治愈。结论面中部脱套入路治疗鼻眶筛区骨折具有瘢痕隐蔽、显露充分的优点,值得推广应用。Objective To evaluate the clinical outcome of midfacial degloving approach in the treatment of naso- orbital-ethmoid（NOE） fracture. Methods Nine cases of NOE fracture patients were treated with open reduction and internal fixation through midfacial degloving approach. Results In all 9 patients, deformities in the NOE region were corrected successfully without leaving an external scar through midfacial degloving approach. There were 3 cases of postoperative complications including 1 case （one side ） of circle stenosis of nasal aperture and 2 cases of facial numbness. Conclusion Midfacial degloving approach offers good exposure for NOE region, specially infraorbital margin, center fragments, nasal bone and piriform aperture without leaving an external scar.
