胚胎移植管血染对IVF/ICSI助孕结局的影响

摘　　要：目的探讨胚胎移植管血染对IVF/ICSI助孕结局的影响。方法回顾性分析单中心行IVF/ICSI,并新鲜周期移植的患者资料。按照移植胚胎时移植管血染与否及血染程度分组,比较各组临床资料、实验室指标及随访结局。结果纳入2 601个IVF/ICSI周期,血染组共847个周期,无血染组1 754个周期。两组患者年龄、不孕年限、基础FSH等基本情况具有可比性,HCG日内膜厚度、获卵数、Gn总量、2PN受精率、移植胚胎数均差异无统计学意义(P>0.05)。血染组种植率30.7%低于无血染组的34.8%(P<0.01);血染组临床妊娠率47.6%低于无血染组的54.1%(P<0.05)。将血染组按血染程度分为少许血染、中量血染和明显血染3组,进行亚组分析显示种植率及临床妊娠率在3组间无明显差异(P>0.05)。单因素logistic回归分析显示移植管血染是影响临床妊娠的危险因素。结论移植管血染可能降低胚胎种植率、临床妊娠率。血染程度与胚胎种植率、临床妊娠率的关系还有待于进一步研究。Objective To evaluate the correlation between blood staining on the embryo transfer catheter and the outcome in fresh IVF / ICSI cycles. Methods Retrospective analysis was performed on the data about patients who underwent fresh long protocol IVF / ICSI cycle in a single IVF center. The cycles were grouped into three according to a semiquantitative system for recording the amount of blood on the transfer catheter. The demographic data,implantation rate, and clinical pregnancy rate were noted and compared. Results A total of 2 601 cycles were examined,among which 1 754 cycles were without blood staining on the catheter during ET,while the other 847 cycles were associated with blood staining. The implantation rate（ IR） and clinical pregnancy rate（ CPR） in the group without blood staining were significantly higher than the group with blood staining（ 34. 8% vs 30. 7%,P = 0. 002; and 54. 1% vs 47. 6%,P value = 0. 013）. However,there was no significant difference was found in IR or CPR among different groups of bloody staining degree. Conclusion Blood staining on the embryo transfer catheter may reduce the embryo implantation and clinical pregnancy rate. However,the degree of bloody staining may not worsen the clinical outcome.
