摘 要:目的了解宫颈癌患者人乳头瘤病毒(HPV)单一感染及多重感染状况。方法采集338例宫颈癌患者的宫颈脱落细胞标本,采用导流杂交基因芯片技术检测HPV基因型;分析宫颈癌不同临床及病理指标与HPV感染的关系。结果宫颈癌患者HPV阳性率为90.53%(306/338);306例阳性标本中HPV单一感染247例占80.72%,多重感染59例占19.28%;HPV单一感染前3位最常见亚型为16型155例占62.75%、18型46例占18.62%、58型12例占4.86%;多重感染前3位最常见亚型为16型40例占67.80%、52型20例占33.90%、58型17例占28.81%。汉族与少数民族HPV阳性率、多重感染率比较,差异无统计学意义(P>0.05)。不同年龄、病理分型、淋巴结转移、肿瘤分期、组织分化患者HPV单一感染率、多重感染率比较,差异均无统计学意义(P>0.05)。结论 HPV多重感染尤其合并多种高危亚型感染可能促进宫颈癌的发生、发展并影响其预后。Objective To investigate the roles of single and multiple human papillomavirus( HPV) infections in patients with cervical cancer. Methods The specimens collected from 338 women with cervical cancer were detected for HPV gene classification analysis by flow-through hybridization and gene chip technology,and the correlation of HPV infection with different clinical and pathological parameters of cervical cancer was analyzed. Results The HPV positive rate was 92. 53%( 306 /338) in cervical cancer patients. Among 306 positive specimens,there were 247 with single HPV infection which accounted for 80. 72%,and 59 with multiple HPV infection which accounted for 19. 28%. The three most common subtypes of single infection were HPV16( 155 cases,accounted for 62. 75%),HPV18( 46 cases,accounted for18. 62%) and HPV58( 12 cases,accounted for 4. 86%). Whereas the three most common subtypes of multiple infection were HPV16( 40 cases,accounted for 67. 80%),HPV52( 20 cases,accounted for 33. 90%) and HPV58( 17cases,accounted for 28. 81%). There was no significant difference in the HPV positive rate and multiple infection rate between Han nationality and minority( P〉0. 05). The single and multiple HPV infection rates showed no significant difference among patients with different clinical and pathological parameters including age,pathological type,lymph node metastasis,TNM stage,tumor differentiation( P〉0. 05). Conclusion The multiple HPV infection,especially complicated with various high-risk subtypes,may affect the occurrence,development and prognosis of cervical cancer.
