两种针刺疗法对周围性面瘫的临床疗效比较

摘　　要：目的:比较以面部阳明经穴为主的传统针刺疗法和以乳突及耳周少阳经穴为主的针刺疗法在治疗周围性面瘫的临床疗效。方法:将90例周围性面瘫患者随机分两组,阳明经穴组45例采用传统的针刺面部阳明经穴为主的方法治疗;少阳经穴组45例采用针刺乳突及耳周少阳经穴为主的治疗;10天为1个疗程,共观察3个疗程;以面部肌肉恢复症状及House-Brackmann分级评分标准为观察指标。结果:两组在治愈率、总有效率均有显著性差异(P<0.05),House-Brackmann分级评分标准,均有显著性差异(P<0.05)。结论:阳明经穴为主的传统针刺疗法优于乳突及耳周少阳经穴为主的针刺疗法,提示针刺治疗周围性面瘫中刺激面部神经末梢穴位而达到面肌功能恢复的机制比刺激神经干和通过针刺改善茎乳突孔内急性非化脓性炎症压迫而达到面肌功能恢复机制更占优势。Objective： To compare the therapeutic effects of acupuncture treatment on peripherial facial paralysis by mainly selecting acupoints of Yangming Meridian located in the face with treatment by mainly selecting acupoints of Shaoyang Meridian located in mastoid and around the ear. Methods： Ninety cases of peripherial facial paralysis were randomly divided into two groups （Yangming Meridian group and Shaoyang Meridian group, each contained forty-five cases）. The Yangming Meridian group （forty-five cases） was mainly treated with acupuncture at Yangming Meridian located in the face. The Shaoyang Meridian group （forty-five cases） was mainly treated with acupuncture at Shaoyang Meridian located in mastoid and around the ear. Every ten-day was a course of treatment, and we observed three courses of treatment. We observed the resumption of facial musle function and House-Brackmann scale after the treatment. Results： The two groups had significant differences in cure rate and total effective rate （P 〈 O. 05 ）. The two groups also had significant differences in House-brackmann scale （ P 〈 0. 05 ）. Conclusion： Treatment on peripherial facial paralysis by mainly selecting acupoints of Yangming Meridian located in the face is better than treatment by mainly selecting acupoints of Shaoyang Meridian located in mastoid and around the ear. It indicates that, for the acupuncture treatment of peripherial facial paralysis, the mechanism of stimulating the nerve ending located in acupoints of the face for improving facial muscle function is more dominant than the mechanism of stimulating nerve trunk and ameliorating the acute non-suppurative inflammation in stylomastoid foraInen by acupuncture.
