青光眼术后白内障术式选择与疗效观察

摘　　要：目的探讨研究透明角膜切口超声乳化术与经颞侧巩膜隧道小切口白内障摘出术治疗青光眼后白内障的临床效果。方法选取医院83例(87眼)青光眼术后白内障患者作为研究对象,分为Ⅰ组42例(44眼)和Ⅱ组41例(43眼),Ⅰ组采用透明角膜切口超声乳化术进行治疗,Ⅱ组采用经颞侧巩膜隧道小切口白内障摘出术治疗。观察2组患者术前术后的眼压水平、角膜内皮计数、术中术后并发症,评价2种术式临床效果。结果Ⅰ组患者视力恢复率为63.64%,Ⅱ组患者视力恢复率为60.47%,2组比较差异无统计学意义(P>0.05);Ⅰ组患者术后出现眼压过高症状4眼,Ⅱ组术后出现眼压过高6眼,经治疗后均可恢复正常;2组患者经治疗后角膜内皮计数指标均得到改善,比较差异无统计学意义(P>0.05);2组患者均出现一定程度的术中术后并发症,发生例数无显著差异。结论透明角膜切口超声乳化术和颞侧巩膜隧道小切口白内障摘出术均为临床治疗青光眼术后白内障的有效方式,应根据患者眼部情况以及实际需求选择合理的手术方式。Objective To observe the clinical effect of clear corneal incision phacoemulsification and through tempo-ral scleral tunnel small incision cataract operation in treatment of cataract after glaucoma surgery. Methods 83 cases of our hospital（87 eyes）with cataract after glaucoma surgery patients were selected as the research object,and were divided into group Ⅰ（44 eyes）and group Ⅱ（43 eyes）. The group Ⅰ was treated with clear corneal incision phacoemulsification,while the group Ⅱ was through temporal scleral tunnel small incision cataract surgery for treatment. The intraocular pressure level, corneal endothelial cell counting,intraoperative and postoperative complications were observed. Results The vision recovery rate of group Ⅰ was 63. 64% ,and the vision recovery rate of group Ⅱwas 60. 47% ,there was no statistically significant（P ﹥0. 05）;There were 4 eyes with ocular hypertension symptoms in group Ⅰ,there were 6 eyes with ocular hypertension symptoms in group Ⅱ,this can be recovered after treatment. The corneal endothelial cell counting indicators of 2 groups improved,There was no significant differences between 2 groups（P ﹥ 0. 05）;Both groups had postoperative complications,and there was no sig-nificant difference in the number of cases. Conclusion Transparent corneal incision phacoemulsification and temporal scleral tunnel small incision cataract operation are all effective way to cure cataract after glaucoma surgery,operation mode should be selected according to the actual needs.
