横结肠代食管在巨大食管平滑肌瘤外科治疗中的应用

摘　　要：目的 总结横结肠代食管治疗巨大食管平滑肌瘤的经验体会.方法 回顾性分析收治的28例巨大食管平滑肌瘤患者的临床资料.结果 28例患者均治愈,无围手术期死亡.术后随访3个月至5年,进食正常,无胸骨后不适,无肿瘤复发,术后颈部吻合口瘘1例,肺部感染2例,乳糜胸1例.结论 横结肠代食管治疗巨大食管平滑肌瘤患者手术效果较好,特别适合已行胃大部切除术后患者的食管重建.Objective To summarize the experience of transverse colon replacing esophagus in the surgical treatment of enormous esophageal leiomyoma.Methods The clinical data of 28 patients with enormous esophageal leiomyoma were retrospectively analyzed.Results The 28 patients were all cured,there was no peroperative period death.The patients were followed up from 3 months to 5 years,eating were normal,no retrostemal discomfort and tumor recurrence.Postoperative cervical part anastomotic stoma was in 1 case,pulmonary infection was in 2 cases,chylothorax was in 1 case.Conclusion Transverse colon replacing esophagus is good for enormous esophageal leiomyoma,especially to the patients who underwent subtotal gastrectomy need esophageal reconstruction.
