摘  要:目的通过测定右旋葡聚硫酸钠(dextran sulfate sodium,DSS)溶液诱导建立的大鼠溃疡性结肠炎(ulcerative colitis,UC)模型尿液中乳果糖、甘露醇、三氯蔗糖的浓度来评估肠道通透性的变化,寻找一种非侵入性检查手段评估UC患者肠黏膜损伤以及通透性的变化。方法 18只健康SD大鼠随机分成两组:UC模型组(UC组,n=12)和正常对照组(NC组,n=6),UC组大鼠自由饮用DSS溶液(浓度为40 g/L)7 d,建立急性UC模型组,NC组正常饮水,两组均正常进食,每天记录大鼠体质量、粪便性状、粪便隐性或显性出血情况。第8天造模成功后予灌注含乳果糖120 mg、甘露醇80 mg、三氯蔗糖60 mg的糖溶液,在代谢笼中收集24 h尿量,高压液相色谱法检测大鼠尿液中糖分子探针排泄量。结果与正常对照组相比,UC大鼠尿中乳果糖排出率显著增高(1.63±0.33 vs 0.055±0.0035,P<0.05),甘露醇排除率下降(0.016±0.0022 vs 0.017±0.0054,P<0.05),乳果糖/甘露醇比值在模型组显著增加(107.50±29.25 vs 3.84±2.29,P<0.05);三氯蔗糖总排出量明显高于正常对照组(0.69±0.13 vs 0.12±0.095,P<0.05),三氯蔗糖/甘露醇比值在模型组显著增加(45.14±9.57 vs 8.40±6.41,P<0.05)。结论 DSS诱导的UC大鼠肠道黏膜屏障受损,肠道通透性增加。检测糖分子探针是一种较好的评估肠黏膜受损及反映肠道通透性改变的非侵入性方法。Objective To investigate the intestinal permeability in DSS-induced rat by rneasuring urinary excretion of orally administered sugars and explore non-invasive methods to assess the intestinal mucosal injury and permeability changes of ulcerative colitis (UC) patients.Methods Eighteen Sprague-Dawley rats were randomly divided into UC group (n=12) and normal control group (NC group,n=6).Rats in UC group were fed 4% DSSfor7 days to induce ulcerative colitis.Rats in NC group were fed tap water.Both groups took normal food.The weight,stool characteristics,and occult or dominant hemorrhage of rats were recorded every day.On 8th day,the rats were removed to the metabolic cages for collecting 24 h urine after intragastric administration the complex solution of the three sugars (the lactulose 120 mg,mannito 180 mg,sucralose 60 mg).The urine was quantitatively analyzed for the three sugars by high pressure liquid chromatography (HPLC).Results Lactulose excreted from the urine of the UC rats was significantly higher (1.63 ±0.33 vs 0.055 ±0.0035,P < 0.05),mannitol excretion rate was lower (0.016 ± 0.0022 vs 0.017 ± 0.0054,P <0.05),lactulose/mannitol ratio (L/M) was significantly increased (107.50 ±29.25 vs 3.84 ± 2.29,P <0.05),the total sucralose excretion was significantly higher (0.69 ± 0.13 vs 0.12 ± 0.095,P < 0.05),sucralose/ mannitol ratio (S/M) was significantly increased (45.14 ± 9.57 vs 8.40 ± 6.41,P < 0.05) compared with the NC group.Conclusion The intestinal mucosal barrier of DSS-induced UC rats is damaged and the intestinal permeability is increased.Detection of sugar molecule probe can be a good method for assessment intestinal mucosal damage and reflect the changes of intestinal permeability.
