缺血性脑血管病合并高血压病患者全脑血管造影特点分析

摘　　要：目的探讨缺血性脑血管病合并高血压病患者全脑血管造影的特点。方法应用德国西门子公司生产的NEUROSTAR PLUS/TOP双C型臂血管造影系统,对417例缺血性脑血管病合并高血压病(高血压组)患者与202例缺血性脑血管病不合并高血压病(对照组)患者的全脑血管造影结果进行比较。结果高血压组与对照组在血管狭窄程度差异有统计学意义(χ2=11.45,P<0.05);高血压组血管异常的发生率明显多于对照组(χ2=10.31,P<0.05)。两组血管病变均以颅内动脉病变为主,最易受累血管为大脑中动脉,差异无统计学意义(χ2=0.06,P>0.05)。两组均以多支血管病变为主,但高血压组多支血管病变的发生率明显高于对照组(66.15%vs50.60%),且以弥漫性血管病变(58.07%)和有侧枝血管代偿(55.21%)为主,而对照组以非弥漫性血管病变和无侧枝血管代偿为主。结论缺血性脑血管病合并高血压病患者全脑血管造影特点为血管狭窄程度更重,以颅内动脉受累为主,常表现为多支血管病变、弥漫性血管病变。Objective To study the characteristics of cerebral angiography in ischemic cerebrovascular patients with hypertension. Methods With NEUROSTAR PLUS / TOP dual C-arm angiography system which produced by the German company Siemens,compare cerebral angiography results between hypertension group（ 417 ischemic cerebrovascular patients who with hypertension） and control group（ 202 ischemic cerebrovascular patients who without hypertension）. Results Vascular stenosis between hypertension group and control group was statistically significant difference（ χ2= 11. 45,P 0. 05）; the incidence of vascular abnormalities was significantly higher in hypertensive group than the control group（ χ2= 10. 31,P 0. 05）. there was no statistically significant difference between two groups in vascular lesions,which mainly involved in intracranial arteries,especially in the middle cerebral artery（ χ2= 0. 06,P 0. 05） Two groups are mainly multivessel disease,but the incidence of multi-vessel disease in hypertensive group was significantly higher than control group（ 66. 15% vs 50. 60%）,and were mainly diffuse vascular lesions（ 58. 07%） and compensatory collateral vessels（ 55. 21%）,while the control group was mainly non-diffuse vascular lesions and no collateral vessels compensatory. Conclusion Cerebral angiography in ischemic cerebrovascular patients who with hypertension is characterized by heavier vascular stenosis,which mainly involved in intracranial artery,usually presents with multi-vessel disease and diffuse vascular disease.
