低位直肠癌经肛尾介入直肠系膜内注射5-氟尿嘧啶缓释剂的新辅助化疗实验研究

摘　　要：目的研究体外经肛尾介入骶前间隙进针,直肠系膜内注射5-氟尿嘧啶(5-Fu)缓释剂对直肠癌肿瘤细胞的诱导凋亡作用。方法建立人直肠癌裸鼠原位种植模型,将荷瘤鼠27只随机分为A、B、C3组。移植后第35天,A组给予直肠系膜内注射5-Fu缓释剂5μl,1mg/kg;B组给予直肠系膜内注射生理盐水5μl;C组经腹腔注射5-Fu缓释剂,1mg/kg。化疗48h后处死裸鼠,测定肿瘤细胞凋亡系数(AI)。结果 A组肿瘤细胞的AI为(9.09±2.86)%、明显高于B组[(1.07±0.43)%]和C组[(4.06±1.00)%],差异有统计学意义(P<0.001);A、B组直肠系膜未见出血、细胞溶解、坏死。结论直肠系膜内注射5-Fu缓释剂对肿瘤细胞有明显的诱导凋亡作用。该途径方便、安全、实用、易推广。Objective In vitro studies by the anus tail intervention before di needle into the gap,and injected the mesenteric 5-fluorouracil （5 its） HuanShiJi of rectal carcinoma of the tumor cell apoptosis induction role.Methods Human carcinoma of colorectal LoVo cell line used in the experiment were cultured in vitro.Orthotopic rectal cancer was established in nude mice with LoVo cells.The mean time of tumor growth was 15.4 d.The rate of tumor formation was 77.14％ （27/35）.Tumor-bearing nude mice were divided randomly into A,B,C 3 groups.In group A（experimental group）,sustained-released 5-Fu was used by mesorectum infusion,1 mg/kg,5 μl;in group B（negative control group）,0.9％ normal saline was used by mesorectum infusion,5 μl;in group C （positive control group）,sustained-released 5-Fu was given intraperitoneally,1 mg/kg.Assayed the apoptosis indexes （AI） of tumor cells after 48 hours of treatment.Apoptotic cells were stained by the terminal deoxynucleotidyl transferesemediated dUTP-biotin nick end labeling （TUNEL） method.The data were expressed as Mean ± SD.Rank sum test was adopted in inter-group comparison,Dunnett T3 was used for comparison between each two groups,all data were analyzed statistically to use SPSS 16.0 for Windows.Results The AIs of tumor cells group A,B,C were （9.09 ±2.86） ％,（1.07 ± 0.43） ％,（4.06 ± 1.00） ％ respectively.There was significant difference among the groups A,B and C（P ＜0.001）.No notably hemorrhage cathepsis and necrosis were found in the Mesorectum of group A and B.Conclusions Sustained-released 5-Fu was able to induce the apoptosis of rectal cancer cells via intra-mesorectum injection.The way is easy,safe,practical and easy to promotion.
