摘 要:目的通过检测14种食物成分的特异性IgG水平,研究食物不耐受与非酒精性脂肪肝、高脂血症的关系。方法选择2011~2013年青岛市1052名健康体检者,应用酶联免疫法(ELISA)检测血清14种食物特异性IgG抗体,采用SPSS 17.0软件对相关资料进行χ^2检验、非条件Logistic回归分析。结果(1)1052名受检人群中牛奶不耐受(χ^2=70.457, P<0.05)、螃蟹不耐受(χ^2=49.040, P<0.05)、海虾不耐受(χ^2=20.692,P<0.05)与非酒精性脂肪肝有关;牛奶不耐受与高脂血症有关(χ^2=24.871,P<0.05)。(2)分别以非酒精性脂肪肝、高脂血症为因变量,年龄、性别、体重指数(BMI)、海鲜和(或)非海鲜类食物不耐受为自变量进行多因素Logistic回归分析,结果显示非酒精性脂肪肝、高脂血症与海鲜、非海鲜类食物均不耐受组及海鲜类食物不耐受、非海鲜类食物耐受组比较,差异有统计学意义, OR 值(95% CI)分别为1.361(1.018~2.057)、1.372(1.040~6.071)、16.976(2.519~125.634)、2.674(1.481~37.087)。结论牛奶、螃蟹、海虾不耐受以及BMI与非酒精性脂肪肝有关,其中牛奶不耐受与非酒精性脂肪肝、高血脂均有关,海产品中螃蟹、海虾不耐受与非酒精性脂肪肝有关;海鲜类食物不耐受较非海鲜类食物不耐受更易导致非酒精性脂肪肝、高脂血症。因此,建议据食物不耐受检测结果加强饮食管理,预防非酒精性脂肪肝、高脂血症的发病。Objective To investigate the association between food intolerance and nonalcoholic fatty liver , hyperuricemia .Methods A total of 1052 Qingdao residents were recruited in this study from 2011 to 2013 .ELISA was used to test specific allergic IgG antibodies .Chi-square test and Logistic regression were used for data analysis by SPSS 17.0 .Results ( 1 ) In the 1052 residents , there were significant difference between food intolerance and nonalcoholic fatty liver:milk intolerance and nonalcoholic fatty liver (χ^2=70.457,P〈0.05),Crab intolerance and nonalcoholic fatty liver (χ^2=49.040 ,P〈0.05 );Prawns intolerance and nonalcoholic fatty liver (χ^2=20.692 , P〈0.05 );There was significant difference between food intolerance and hyperlipidemia : milk intolerance and hyperlipidemia(χ^2=24.871,P〈0.05);(2)In logistic regression analysis,there was significant difference between nonalcoholic fatty liver and sea food intolerance ,non sea food intolerance ,the odds ratio was 1.361(1.018-2.057);Nonalcoholic fatty liver and sea food intolerance ,non sea food intolerance:1.372 (1.040-6.071 );Hyperlipidemia and sea food intolerance,non sea food tolerance:16.976(2.519-125.634);Hyperlipidemia and sea food intolerance ,non sea food tolerance:2.674 (1.481-37.087).Conclusion There is correlation between milk,crab,eggs,Prawns intolerance ,BMI and nonalcoholic fatty liver ,hyperuricemia;The risk of suffering nonalcoholic fatty liver is larger in sea food intolerance than non sea food;Sea food intolerance result in nonalcoholic fatty liver , hyperlipidemia more easily than non sea food .Therefore , food adjustment might be useful in the prevention and treatment of metabolic disease according to the test results the food intolerance .
