盐酸去甲万古霉素结膜下注射的眼表安全性评估

摘　　要：目的 评估内眼手术后结膜下注射盐酸去甲万古霉素的眼表安全性.方法 采用前瞻性非随机对照临床试验,将64例（均为单眼）内眼手术后患者分为A、B两组,A组结膜下注射10mg去甲万古霉素（n=33）,B组结膜下注射20mg去甲万古霉素（n=31）,比较观察A、B两组术后第一天及术后一周的眼表情况.结果 术后第一天A、B两组均出现结膜下水肿及出血,B组发现15例结膜下黄白色颗粒状药物残留.其结膜下水肿高度分级1级A组24例,B组14例,2级A组8例,B组13例,3级A组1例,B组4例,A组结膜下水肿程度比B组轻（P＜0.05）；结膜下水肿面积A组（74.58±20.050）mm2,B组（89.42±16.998）mm2,两组间比较差异有统计学意义（P＜0.05）；结膜下出血面积A组（75.97±16.423）mm2,B组（87.32±13.828）mm2,两组间比较差异有统计学意义（P＜0.05）.术后一周A、B两组结膜下水肿消失,结膜下出血明显减轻,B组结膜下颗粒状药物完全吸收.结论 去甲万古霉素结膜下注射眼表安全,10mg眼表安全性更高.Objective Present study was to assess the ocular surface safety of Subconjunctival injecting Norvancomyein Hydrochloride after intraocular surgery. Methods A prospective non-random clinical study was performed in 64 cases（oeellanae）, who received intraocular surgery in our hospital. Patients were divided into A and B group. A group was Subconjunctival injected 10rag Norvancomycin（n= 33）, B group was injected 20mg（n=31）. The ocular surface of two groups were observed in the first day and one week after intraocular surgery. Results Two groups occurred subconjunctival edema and Subconjunctival hemorrhage in the first day, and B group appeared 15 cases Subconjunctival medicine residue who were yellow and white. The height of subconjunctival edema was divide 3 level. 1 level included 24 cases in the A group and 14 cases in the B group ;2 level include 8 cases in the A group and 13 cases in the B group;3 level included 1 case in the A group and 4 cases in the B group. A group was slighter than B in the degree of subconj unctivaI edema （P〈0.05）. The area of subconjunetival edema was （ 74.58 ± 20. 050） mm2 and （89.42±16. 998）mm^2 respectively in A and B group,and was statistically significant difference these two groups（P %0.05）. The area of subconjunctival hemorrhage was（75. 97±16. 423）mm2 and（87. 32±13. 828）mm2 respectively in A and B group, and was statistically significant difference these two groups（P〈0.05）. The subconjunctival edema of two groups was vanished and subconjunetival hemorrhage was conspicuous lessened after one week,moreover,Subconjunctival medicine residue was completely absorbed. Conclusion The ocular surface of Subconjunetival injecting Norvancomycin Hydroehloride was safe and the safety was more better when was injected 10mg Norvaneomyein.
