摘 要:目的探讨纳米炭为载体的淋巴靶向化疗对结肠淋巴结内转移癌细胞凋亡的影响。方法随机将2009年1月至2012年6月术前通过CT判断有或可能有淋巴转移的45例结肠癌病例随机分为:淋巴化疗组23例,术前72~48h在纤维结肠镜下,将纳米炭-氟尿嘧啶(CNP-5-FU)混合液多点注射于距肿瘤边缘约1.0cm的正常结肠黏膜下。常规行结肠癌根治手术,术后整理切除标本,淋巴结分组,将取到的淋巴结分成2份,深低温冰箱保存备用。对照组22例,常规行结肠癌根治手术和标本整理,淋巴结保存同淋巴化疗组。术后对两组有淋巴转移的病例应用流式细胞分析法检测淋巴结内转移癌细胞的凋亡率,应用原位末端脱氧核苷转移酶标记法(TUNEL法)检测淋巴结内转移癌细胞的凋亡指数。结果淋巴化疗组和对照组淋巴结内转移癌细胞凋亡率分别为(17.36±2.89)%和(1.40±0.40)%,淋巴结内转移癌细胞凋亡指数分别为(32.84±3.14)%和(13.25±2.67)%,两组比较有统计学差异(P<0.05)。结论载药纳米炭能够靶向性进入淋巴结内,有效诱导淋巴结内转移癌细胞的凋亡,对淋巴结转移灶具有直接治疗作用。Objective To investigate the effect of activated carbon nanoparticles targeted chemotherapy on the colon cancer metastatic cellular apoptosis in the lymph node. Methods 45 cases of colon cancer were divided into two groups randomly. Experimental group ( n = 23 ) : 72-48 hours before radical colon cancer resection, carbon nanoparticles-fluorouracil (CNP-5-FU) mixture was multi-point injected in normal colonic mucosa about 1.0 cm from the edge of the tumor by fibro-eolonoseope. All of the lymph nodes were divided equally and restored in deep cryogenic refrigerator. Control group ( n = 22 ) : After routinely radical colon cancer resection, lymph nodes were restored as the Experimental group. The rate of apoptosis of metastatic cells in lymph nodes was analyzed by Flow cytometry. The cellular apoptosis index of metastatic tumor cells in lymph nodes were tested by TUNEL assay. Results The apoptosis rate of metastatic cell of Experimental group and Control group were ( 17. 36 ± 2. 89 ) % and (1.40 ±0. 40)% ,respectively. Apoptosis index of metastatic cancer cell in lymph nodes were (32. 84 ± 3.14)% and ( 13.25 ± 2. 67 ) %, respectively. The differences were significant ( both P 〈 0. 05 ). Conclusions Drug- loading carbon nanoparticles can be imported and remained in lymph node, effectively induce metastatic cell apoptosis in lymph node and directly treat the metastatic cancer in the lymph node.
